摘　　要：目的探讨甘油三酯-葡萄糖(TyG)指数对糖尿病视网膜病变(DR)患者激光治疗转归的预测效能,以期为临床治疗决策的制定提供参考。方法选取2022年1月—2024年5月在四川某医院就诊并接受激光治疗的150例DR患者作为研究对象,根据所有患者激光治疗后的不同转归情况设改善组和未改善组。所有患者激光治疗前均行血液生化检测并记录临床资料。绘制受试者工作特征(ROC)曲线评价TyG指数预测DR患者激光治疗转归的价值,并探讨DR患者激光治疗转归的影响因素。结果未改善组患者TyG指数、甘油三酯(TG)高于改善组,差异均有统计学意义(P<0.05)。单因素logistic回归分析结果显示:DR患者激光治疗前的TyG指数是激光治疗转归的影响因素(P<0.05)。多元logistic回归分析结果显示:DR患者激光治疗前的TyG指数和合并高血压是DR患者激光治疗转归的影响因素(P<0.05)。ROC曲线显示,激光治疗前TyG指数预测DR患者激光治疗转归的曲线下面积(AUC)(95%CI)为0.912(0.867~0.962),最佳截点值为9.12。结论TyG指数对DR患者激光治疗转归具有较好的预测效能,其值升高可增加激光治疗预后不佳风险,可作为DR患者激光治疗转归的生物学标志物。Objective To investigate the predictive efficacy of the triglyceride-glucose(TyG)index on laser treatment outcomes in patients with diabetic retinopathy(DR),providing a reference for clinical decision-making.Methods A total of 150 DR pa-tients who underwent laser treatment at a hospital in Sichuan from January 2022 to May 2024 were selected as the study partici-pants.Based on post-treatment outcomes,the patients were assigned to an improved group and a non-improved group.Blood biochemical tests and clinical data were collected before laser treatment.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive value of the TyG index for laser treatment outcomes,and influencing factors were explored.Results The TyG index and triglyceride(TG)levels in the non-improved group were significantly higher than those in the im-proved group(P<0.05).Univariate logistic regression showed that the pre-treatment TyG index was a significant factor influ-encing laser treatment outcomes(P<0.05).Multivariate logistic regression indicated that the pre-treatment TyG index and comorbid hypertension were independent influencing factors for laser treatment outcomes(P<0.05).The ROC curve demon-strated that the area under the curve(AUC)(95%CI)of the TyG index for predicting laser treatment outcomes was 0.912(0.867-0.962),with an optimal cutoff value of 9.12.Conclusion The TyG index exhibits strong predictive efficacy for laser treatment outcomes in DR patients.Elevated TyG index increases the risk of poor prognosis after laser treatment and may serve as a biomarker for evaluating treatment outcomes in this population.
