摘  要:目的探讨钉棒系统固定Tile C1型骨盆骨折有限元模型中骶髂关节界面不同设置类型对骨盆生物力学的影响。方法获取1名健康青年男性骨盆三维CT数据,通过计算机软件建立Tile C1型骨盆骨折有限元模型(模型A)、钉棒系统固定后患侧骶髂关节界面非接触模型(模型B)、钉棒系统固定后患侧骶髂关节界面绑定约束模型(模型C)。对髋臼的6个自由度进行固定,在骶骨上表面施加垂直向下500 N的载荷以模拟双腿站立状态下受力情况,比较模型B与模型C的位移和应力分布差异。结果模型B与模型C最大位移均出现在骶骨最末端,模型B最大位移大于模型C。模型B髂骨最大位移出现在健侧髂骨翼后上部和骶髂关节处,前后方内固定最大位移均出现在健侧螺钉与连接杆结合处,而模型C最大位移出现在患侧相应位置。模型B骨盆最大应力出现在健侧坐骨大切迹附近,而模型C出现在患侧坐骨大切迹附近,模型B骨盆最大应力大于模型C。模型B前后方内固定最大应力均发生在健侧螺钉与髂骨结合处,模型C前后方内固定最大应力均发生在患侧螺钉与髂骨结合处。结论钉棒系统固定Tile C1型骨盆骨折有限元模型中骶髂关节界面构建方法影响骨盆和内固定的应力和位移分布,患侧骶髂关节复位和加压效果越好,界面越稳定,骨盆整体稳定性越高,内固定失效风险越小。ObjectiveTo investigate the effect of sacroiliac joint interface setting on pelvic biomechanics in finite element model of Tile type C1 pelvic fractures fixed by screw rod system.MethodsThree-dimensional CT data of the pelvis of a young healthy male were obtained.The finite element software was used to establish a Tile type C1 pelvic fracture model(Model A),a non-contact model of the sacroiliac joint interface on the injured side after fixation with the screw rod system(Model B),and a bound constraint model of the sacroiliac joint interface on the injured side after fixation with the screw rod system(Model C).The six degrees of freedom of the acetabulum were fixed,and a vertical downward load of 500 N was applied to the upper surface of the sacrum to simulate the force situation when the legs were standing.The differences in displacement and stress distribution were compared between sacroiliac joint bound constraint and non-contact models.ResultsThe maximum displacements of models B and C both appeared at the extreme end of the sacrum,and the maximum displacement of model B was greater than that of model C.The maximum displacement of the ilium of model B occurred at the upper and posterior part of the iliac wing and the sacroiliac joint on the healthy side.The maximum displacement of the anterior and posterior internal fixation occurred at the junction of the screw and the connecting rod on the healthy side,while the maximum displacement of model C occurred at the corresponding location on the injured side.The maximum stress in the pelvis of model B appeared near the greater sciatic notch on the healthy side,while that of model C appeared near the greater sciatic notch on the injured side.The maximum stress of model B was greater than that of model C.The maximum stress of the anterior and posterior internal fixation of Model B occurred at the junction of the screw and the iliac crest on the injured side.The maximum stress of the anterior and posterior internal fixation of Model C occurred at the junction of the s
