摘 要:目的分析滑膜软骨瘤病患者术前的高频超声与MRI图像特征,比较二者的诊断效能,为临床合理选择影像学检查方法提供依据。方法纳入自2019-05—2024-04初诊考虑为滑膜软骨瘤病的51例患者,经病理检查明确32例为滑膜软骨瘤病,19例并非滑膜软骨瘤病。观察滑膜软骨瘤病患者的高频超声图像与MRI影像特点,比较高频超声与MRI诊断滑膜软骨瘤病的效能。结果32例滑膜软骨瘤病中,高频超声正确诊断23例,误诊9例;MRI正确诊断22例,误诊10例;二者联合正确诊断30例,误诊2例。19例非滑膜软骨瘤病中,高频超声正确诊断15例,MRI正确诊断16例,二者联合正确诊断17例。高频超声诊断滑膜软骨瘤病的敏感度、特异度、阳性预测值、阴性预测值和符合率分别为71.9%、78.9%、85.2%、62.5%、74.5%。MRI诊断滑膜软骨瘤病的敏感度、特异度、阳性预测值、阴性预测值和诊断符合率分别为68.9%、89.5%、88.0%、63.0%、76.5%;二者联合诊断的敏感度、特异度、阳性预测值、阴性预测值和符合率分别为93.8%、89.5%、93.8%、89.5%、91.2%。高频超声与MRI诊断滑膜软骨瘤病的符合率差异无统计学意义(P>0.05),二者联合的诊断符合率高于高频超声、MRI(P<0.05)。结论高频超声与MRI联合应用能提高滑膜软骨瘤病的诊断正确率,为临床治疗提供更为可靠的依据。高频超声作为基本筛查方法,MRI作为补充,有利于精确评估病变范围、严重程度以及骨骼和软骨受累情况。ObjectiveTo analyze the preoperative imaging features of high-frequency ultrasound and MRI in patients with synovial chondromatosis,compare the diagnostic efficacy of these two methods,and provide a basis for the rational selection of imaging techniques in clinical practice.Methods May 2019 to April 2024 were enrolled.Pathological examination confirmed 32 cases as synovial chondromatosis and 19 cases as non-synovial chondromatosis.The high-frequency ultrasound and MRI imaging features of the patients with synovial chondromatosis were observed,and the diagnostic efficacy of these two methods was compared.ResultsAmong the 32 cases of synovial chondromatosis,high-frequency ultrasound correctly diagnosed 23 cases,and misdiagnosed 9 cases;MRI correctly diagnosed22 cases,and misdiagnosed 10 cases;the combination of both methods correctly diagnosed 30 cases and misdiagnosed 2 cases.Among the 19 non-synovial chondromatosis cases,high-frequency ultrasound,MRI and the combined method correctly diagnosed 15,16 and 17 cases respectively.The sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accordance rate of high-frequency ultrasound for synovial chondromatosis were 71.9%,78.9%,85.2%,62.5%,and 74.5%,respectively.For MRI,these values were 68.9%,89.5%,88.0%,63.0%,and 76.5%,respectively.For the combined method,these values were 93.8%,89.5%,93.8%,89.5%,and 91.2%,respectively.There was no statistically significant difference between high-frequency ultrasound and MRI in terms of diagnostic accordance rate(P>0.05),but the diagnostic accordance rate of the combined method was higher than that of either high-frequency ultrasound or MRI(P<0.05).ConclusionHigh-frequency ultrasound combined with MRI can improve the diagnostic accuracy of synovial chondromatosis,providing a more reliable basis for clinical treatment.High-frequency ultrasound,as a basic screening method,and MRI,as a supplement,are beneficial for the accurate assessment of the extent and severity of the lesion,as well as its bone an
