摘 要:目的:分析多层螺旋CT与MRI影像检查技术在腰椎骨折患者中的诊断价值。方法:回顾性分析2024年1月~2024年6月常州市武进中医医院收治的251例腰椎骨折患者的临床资料,所有患者在入院时实施计算机体层扫描(CT)及磁共振成像(MRI),以外科手术病理结果为“金标准”,对比CT、MRI单独及联合诊断腰椎骨折分型的准确率、椎体及附件骨骨折数量和软组织损伤检出率。结果:CT、MRI联合使用的诊断结果与外科手术结果相比一致性比两者单独使用高,Kappa值为0.606(P<0.05);CT联合MRI检查技术对骨折分型的诊断准确性分别为94.7%、93.9%和96.1%(P<0.05);CT、MRI单独及联合使用诊断椎体骨折、附件骨折和骨片数量的准确率依次递增,最高的为联合使用两种检查技术,分别为96.5%、95.6%和89.6%(P<0.05)。结论:在腰椎骨折中联合使用MRI及CT检查技术可以显著提高骨折分型、软组织损伤和椎体及附件骨骨折数量上诊断准确性。Objective:To evaluate the diagnostic value of combined multilayer spiral CT and MRI imaging in lumbar spine fractures.Methods:A retrospective analysis was conducted on 251 lumbar spine fracture patients admitted to Changzhou Wujin Hospital of Traditional Chinese Medicine from January 2024 to June 2024.All patients underwent CT and MRI upon admission,with surgical pathology serving as the“gold standard.”We compared the accuracy of lumbar fracture classification,detection rates of vertebral and accessory fractures,and soft tissue injuries using CT,MRI,and their combination.Results:The combined CT and MRI approach showed greater consistency with surgical findings(Kappa=0.606,P<0.05)compared to either modality alone.Diagnostic accuracies for fracture classification using combined CT and MRI were 94.7%,93.9%,and 96.1%,respectively(P<0.05).The combined method also demonstrated the highest accuracy in detecting vertebral fractures,accessory fractures,and bone fragment count,at 96.5%,95.6%,and 89.6%,respectively(P<0.05).Conclusion:Combining MRI and CT significantly enhances diagnostic accuracy for fracture classification,soft tissue injury,and detection of vertebral and accessory bone fractures in lumbar spine injuries.
