摘 要:目的:研究数字X射线摄影(DR)全脊柱和全下肢全景成像技术应用在骨科中的临床应用意义。方法:选择2021年1月~2023年1月在福建医科大学附属龙岩第一医院接受全脊柱以及全下肢检查的60例患者,均实施DR全脊柱和全下肢全景成像技术进行检查,然后将病理检查作为金标准,对其影像学结果进行记录分析,比较两种方法的诊断准确率。结果:通过全脊柱检查的患者共有45例,其中包括脊柱侧弯的患者共22例,脊柱侧弯矫正手术治疗的患者共6例,骨质增生共有11例,强直性脊柱炎患者共有5例,未见异常患者共1例;通过全下肢检查的患者共有15例,其中包括胫骨平台骨折畸形愈合患者共3例,股骨颈骨折患者共有3例,髋关节置换术治疗的患者共3例,股骨头发育异常共有1例,先天性股骨头发生缺失共1例,股骨颈骨折手术治疗患者共1例,膝关节骨关节炎患者共3例。DR全脊柱与全下肢全景成像技术的诊断准确率和病理检查相比无显著性差异(P>0.05)。结论:DR全脊柱和全下肢全景成像技术应用在临床骨科患者的诊断中,能够将全脊柱、全下肢完整显示,并且能够对人体脊柱、下肢的正常状态的应力状况进行清晰显示,可为骨科的诊治提供有力依据。Objective:To study the clinical significance of DR full spine and full lower limb panoramic imaging technology in orthopedics.Methods:60 patients who underwent full spine and full lower limb examinations at Longyan First Hospital Affiliated to Fujian Medical University from January 2021 to January 2023 were selected.DR full spine and full lower limb panoramic imaging technology was used for examination,and pathological examination was used as the gold standard to record and analyze the imaging results.The diagnostic accuracy of the two methods was compared.Results:A total of 45 patients underwent total spinal examination,including 22 patients with scoliosis,6 patients with scoliosis correction surgery,11 patients with osteoporosis,5 patients with ankylosing spondylitis,and 1 patient without abnormalities;A total of 15 patients underwent full lower limb examination,including 3 patients with tibial plateau fracture malunion,3 patients with femoral neck fracture,3 patients with hip replacement surgery,1 patient with femoral head dysplasia,1 patient with congenital femoral head loss,1 patient with femoral neck fracture surgery,and 3 patients with knee osteoarthritis.The diagnostic accuracy and pathological examination of DR whole spine and whole lower limb panoramic imaging technology were compared(P>0.05).Conclusion:The application of DR full spine and full lower limb panoramic imaging technology in the diagnosis of clinical orthopedic patients can fully display the entire spine and all lower limbs,and can clearly display the normal stress status of the human spine and lower limbs,providing strong basis for the diagnosis and treatment of orthopedics.
