摘 要:目的分析重型手足口病患儿外周血及脑脊液HMGB1、25(OH)D表达及与脑损伤关系。方法选择医院2018年3月至2021年3月诊治的重症手足口病患儿60例,将其纳入病例组。另选择医院2018年3月至2021年3月非手足口病患儿60例,将其纳入对照组。检查外周血及脑脊液HMGB1、25(OH)D表达及与脑损伤。比较两组患儿外周血及脑脊液HMGB1、25(OH)D水平,分析病例组不同脑损伤患儿外周血及脑脊液HMGB1、25(OH)D水平,探讨其相关性。结果病例组患儿外周血和脑脊液HMGB1水平高于对照组(P<0.05),25(OH)D水平低于对照组(P<0.05)。重度脑损伤患儿外周血及脑脊液HMGB1水平均高于无、轻度和中度患儿(P<0.05),其水平最高;25(OH)D水平均低于无、轻度和中度患儿(P<0.05),其水平最低。HMGB1表达水平与脑损伤程度呈正相关(P<0.05),25(OH)D表达与脑损伤呈负相关(P<0.05)。结论重型手足口病患儿外周血及脑脊液HMGB1、25(OH)D表达与脑损伤关系密切,其中HMGB1会促使脑损伤加重,而25(OH)D会减少脑损伤。Objective To investigate the expression of high-mobility group box 1(HMGB1)and 25 hydroxyvitamin D[25(OH)D]in peripheral blood and cerebrospinal fluid of children with severe hand-foot-mouth disease and their relationship with brain injury.Methods Sixty children with severe hand-foot-mouth disease and 60 children with non-hand-foot-mouth disease in our hospital from March 2018 to March 2021 were enrolled,and set as case group and control group,respectively.Expression levels of HMGB1 and 25(OH)D in peripheral blood and cerebrospinal fluid,and the brain injury were detected.Then the levels of HMGB1 and 25(OH)D in peripheral blood and cerebrospinal fluid were compared between and within groups,thereafter,its relationship with brain injury was discussed.Results Peripheral blood and cerebrospinal fluid HMGB1 level was higher in case group than in control group(P<0.05),and 25(OH)D level was lower in case group than in control group(P<0.05).Compared with children with hand-foot-mouth disease of mild and moderate degrees,HMGB1 level was the highest in server children(P<0.05),and 25(OH)D level was the lowest in severe children(P<0.05).HMGB1 expression levels were positively correlated with the degree of brain injury(P<0.05),and 25(OH)D expression was negatively correlated with brain injury(P<0.05).Conclusion Peripheral blood and cerebrospinal fluid HMGB1 and 25(OH)D of children with severe Hand-foot-mouth disease are strongly associated with brain injury,in which HMGB1 may aggravate the brain injury,while 25(OH)D may alleviate the brain injury.
