摘　　要：目的探讨基于eCASH理念在非小细胞肺癌胸腔镜术后重症镇静镇痛患者中的应用效果。方法选取2021年2月到2023年2月非小细胞肺癌胸腔镜术后入住重症监护室的患者130例,按随机数字表法分为观察组和对照组各65例;其中对照组使用常规术后管理方法,观察组采用eCASH术后管理方法,观察两组的镇静镇痛效果包括CPOT评分、RASS评分、镇静达标时间、停用镇静药后清醒时间以及不良事件发生率。结果观察组VAS评分和RASS评分比对照组低,镇静达标时间和停用镇静药物后清醒时间比对照组短(P<0.05);观察组患者发生谵妄、镇静过度、心动过缓、血压降低的例数少于对照组,总概率低于对照组(P<0.05)。结论eCASH理念应用在非小细胞肺癌胸腔镜术后能起到良好镇静镇痛效果,且不良反应发生率低,值得临床推广。Objective To explore the application effect of eCASH concept in severe sedation and analgesia patients after thoracoscopic surgery for non-small cell lung cancer.Methods 130 patients with non-small cell lung cancer admitted to the intensive care unit after thoracoscopic surgery from February 2021 to February 2023 were selected and randomly divided into an observation group and a control group with 65 cases each;The control group used routine postoperative management methods,while the observation group used eCASH postoperative management methods.The sedative and analgesic effects of the two groups were observed,including CPOT score,RASS score,sedative compliance time,wake-up time after discontinuing sedative drugs,and incidence of adverse events.Results The VAS score and RASS score of the observation group were lower than those of the control group,and the time to achieve sedation and the time to wake up after discontinuing sedative drugs were shorter than those of the control group(P<0.05);The observation group had fewer cases of delirium,excessive sedation,bradycardia,and decreased blood pressure compared to the control group,and the overall probability was lower than that of the control group(P<0.05).Conclusion The application of eCASH concept in thoracoscopic surgery for non-small cell lung cancer can achieve good sedative and analgesic effects,with a low incidence of adverse reactions,and is worthy of clinical promotion.
