摘 要:目的探讨磁共振定量磁化率成像(QSM)鉴别诊断半月板撕裂与半月板变性的价值。方法回顾性分析,收集2022年1月—2023年6月在医院行磁共振检查的半月板撕裂患者40例临床资料,作为半月板撕裂组,并收集同期在医院行磁共振检查的半月板变性患者40例临床资料,作为半月板变性组。所有患者资料均完整,比较两组患者的磁敏感值情况,重点分析磁共振QSM对半月板撕裂与半月板变性的鉴别诊断价值。结果半月板变性组的磁共振磁敏感值比半月板撕裂组高(P<0.05);绘制ROC曲线,磁共振QSM鉴别诊断半月板撕裂与半月板变性的AUC=0.809,AUC的95%CI为0.714~0.904,灵敏度为0.700,特异度为0.800,约登指数为0.600,cut-off值为0.100ppm,鉴别诊断价值理想。结论半月板撕裂、半月板变性使用磁共振QSM鉴别诊断的价值理想,有较高的灵敏度及特异度,可通过测定磁敏感值来对半月板损伤的类型进行鉴别诊断。Objective To explore the value of quantitative magnetic resonance imaging(QSM)in the differential diagnosis of meniscus tear and meniscus degeneration.Methods A retrospective analysis was conducted to collect clinical data of 40 patients with meniscus tear who underwent magnetic resonance imaging in hospitals from January 2022 to June 2023 as the meniscus tear group,and 40 patients with meniscus degeneration who underwent magnetic resonance imaging in hospitals during the same period as the meniscus degeneration group.All patient data are complete,and the magnetic sensitivity values of the two groups of patients are compared,with a focus on analyzing the differential diagnostic value of magnetic resonance QSM for meniscus tear and meniscus degeneration.Results The magnetic resonance sensitivity value of the meniscus degeneration group was higher than that of the meniscus tear group(P<0.05);Draw ROC curve,the AUC of magnetic resonance QSM for differential diagnosis of meniscus tear and meniscus degeneration is 0.809,the 95%CI of AUC is 0.714~0.904,the sensitivity is 0.700,the specificity is 0.800,the Jordan index is 0.600,and the cut-off value is 0.100ppm.The differential diagnostic value is ideal.Conclusion The value of using magnetic resonance QSM for differential diagnosis of meniscus tear and meniscus degeneration is ideal,with high sensitivity and specificity.The type of meniscus injury can be distinguished by measuring the magnetic sensitivity value.
