基于计划行为理论的机械排痰对ICU机械通气患者气道廓清的影响

摘　　要：目的探讨基于计划行为理论(TPB)的机械排痰对重症监护病房(ICU)机械通气患者气道廓清的影响。方法选取2021.9—2024.3本院收治的80例ICU机械通气患者,采用随机数字表法将其分为实验组和对照组各40例。实验组接受基于TPB的机械排痰,对照组接受常规机械排痰。结果在干预措施实施后,实验组患者的排痰难度得分显著低于对照组,差异具有统计学意义(P<0.05),而痰液性质和颜色比较,并未有统计学上的显著差异(P>0.05);两组患者干预前后实验组的PaO_(2)、PaO_(2)/FiO_(2)高于对照组,PaCO_(2)低于对照组(P<0.05);干预后实验组的FVC、FEV1、PEF均高于对照组(P<0.05)。结论基于TPB的机械排痰应用于ICU机械通气患者中,可以有效减少机械通气患者的排痰难度,并显著提升肺部通气功能和全身氧合状态。Objective To explore the effects of mechanical sputum extraction based on the planned behavior theory(TPB)on airway clearance in mechanically ventilated patients in the intensive care unit(ICU).Methods Eighty patients mechanically ventilated in ICU admitted to 2021.9-2024.3 were selected as the study subjects,and the patients were divided into 40 patients in experimental and control groups.The experimental group received mechanical sputum extraction based on TPB,and the control group received conventional mechanical sputum extraction.Results After the intervention of the patients,the difficulty scores were significantly lower than the control group and this difference was statistically significant(P<0.05).However,no statistically significant difference was found between the sputum groups(P>0.05);In both groups,PaO_(2)、PaO_(2)/FiO_(2) and the PaCO_(2) were lower than the control group(P<0.05);FVC,FEV 1,and PEF in the experimental group were higher than those in the control group(P<0.05).Conclusion The TPB-based mechanical sputum discharge applied in mechanically ventilated patients in ICU can effectively reduce the difficulty of sputum discharge in mechanically ventilated patients and significantly improve pulmonary ventilation function and systemic oxygenation status.
