摘 要:目的:观察四君子汤对肝硬化腹水大鼠模型“肠-肝轴”的影响并探讨其作用机制。方法:将60只雄性SD大鼠随机分为正常组、模型组、多烯磷脂酰胆碱胶囊组、双歧杆菌三联活菌散组、四君子汤低剂量组和四君子汤高剂量组,每组10只。正常组模拟造模过程,其余各组建立肝硬化腹水模型,8周后各组给予相应药物灌胃干预,1次/d。4周给药结束后,比较各组大鼠的腹水量及肝脏指数,观察肝脏病理学表现,采用ELISA法检测并比较各组血清前白蛋白(prealbumin, PA)、脂多糖(lipopolysaccharide, LPS)含量;采用免疫组化法检测并比较各组大鼠的大肠组织紧密连接蛋白(Claudin-1)的表达水平。结果:(1)四君子汤低剂量组大鼠的腹水量显著少于模型组(P<0.05),肝脏指数显著高于模型组(P<0.05),四君子汤高剂量组大鼠的腹水量显著少于模型组(P<0.01),肝脏指数显著高于模型组(P<0.01);(2)四君子汤高剂量组大鼠的血清PA、Claudin-1表达水平显著高于模型组(P<0.05),血清LPS水平显著低于模型组(P<0.01)。结论:四君子汤可以对肝硬化腹水大鼠模型的“肠-肝轴”状态产生影响,增强肠道屏障功能,减轻肝脏实质损伤,其作用机制可能和增加大肠组织Claudin-1的表达、修复肠道黏膜机械屏障,从而降低血清LPS对肝脏的二次打击有关。Objective To observe the effect of Sijunzi decoction on the"intestinal-liver axis"in a rat model of liver cirrhosis ascites and explore its mechanism of action.Methods Sixty male SD rats were randomly divided into normal group,model group,polyene phosphatidylcholine capsules group,bifidobacterium triple active powder group,low-dose group of Sijunzi decoction,and high-dose group of Sijunzi decoction,with 10 rats in each group.The normal group simulated the modeling process,while the other groups established a model of liver cirrhosis ascites.After eight weeks,each group was given corresponding drugs for gastric intervention,once a day.After four weeks of administration,the ascites volume and liver index of each group of rats were compared,and the pathological manifestations of the liver were observed.ELISA was used to detect and compare the levels of serum prealbumin(PA)and lipopolysaccharide(LPS)in each group;immunohistochemistry was used to detect and compare the expression levels of the tight junction protein(Claudin-1)in the large intestine tissues of each group.Results①The ascites volume of the low-dose group of Sijunzi decoction rats was significantly lower than that of the model group(P<0.05),and the liver index was significantly higher than that of the model group(P<0.05);the ascites volume of the high-dose group of Sijunzi decoction rats was significantly lower than that of the model group(P<0.01),and the liver index was significantly higher than that of the model group(P<0.01);②The serum prealbumin content and Claudin-1 expression level in the high-dose group of Sijunzi decoction were significantly higher than those in the model group(P<0.05),while the serum LPS content was significantly lower than that in the model group(P<0.01).Conclusion Sijunzi decoction can have an impact on the"intestinal-liver axis"state of a rat model of liver cirrhosis ascites,enhance intestinal barrier function,and reduce liver parenchymal damage.Its mechanism of action may be related to increasing the expression of the tig
