摘 要:目的本研究旨在探究45岁及以上中老年人群握力与代谢异常表型对死亡率的影响,分析握力与代谢状态随时间变化的相互关联及其对死亡风险的潜在影响。方法利用中国健康与养老追踪调查(CHARLS)的纵向队列数据,纳入了8455例研究对象。采用Cox回归分析,计算了不同握力与代谢表型对全因死亡风险的风险比(HR)及95%置信区间(CI)。此外还分析了握力和代谢状态从基线到后续调查的转变对全因死亡的影响(n=5659)。结果与握力和代谢均正常的个体相比,握力正常代谢异常、握力降低代谢正常和握力降低代谢异常的个体HR(95%CI)分别为1.16(1.01~1.34)、1.89(1.57~2.28)和2.11(1.76~2.52)。本研究还发现握力和代谢状态的恶化,显著增加了死亡的风险。结论本研究揭示了握力与代谢异常表型对中老年人群死亡风险的显著影响,强调了动态监测握力与代谢状态的重要性。Objective The aim of this study was to investigate the effects of handgrip strength and metabolic abnormalities on mortality in middle-aged and elderly people aged 45 years and above,and to analyze the correlation between grip strength and metabolic status over time and their potential impact on mortality risk.Methods Using longitudinal cohort data from the China Health and Retirement Follow-up Survey(CHARLS),8455 research subjects were included in the study.Cox regression analysis was used to calculate the hazard ratio(HR)and 95%confidence interval(CI)for the risk of all-cause mortality for different handgrip strength and metabolic phenotypes.In addition,the effects of changes in handgrip strength and metabolic status from baseline to subsequent investigations on all-cause mortality were analyzed(n=5659).Results Compared with individuals with normal handgrip strength and normal metabolism,the total adjusted HR(95%CI)for individuals with normal handgrip strength and abnormal handgrip strength,normal handgrip strength and abnormal handgrip strength were 1.16(1.01~1.34),1.89(1.57~2.28),and 2.11(1.76~2.52),respectively.This study found that worsening handgrip strength and metabolic health significantly increased the risk of death.Conclusion This study reveals the significant influence of handgrip strength and metabolic abnormalities on mortality risk in middle-aged and elderly population,and emphasizes the importance of dynamic monitoring of handgrip strength and metabolic status.
