摘　　要：目的系统评价老年高血压患者在老年综合评估干预下血压等指标的控制效果。方法利用计算机检索The Cochrane Library、PubMed、Embase、Web of Science、中国生物医学文献数据库(CBM)、维普(VIP)、中国知网(CNKI)和万方数据库,查找有关高血压患者进行老年综合评估的研究,检索时限为从数据库建库开始至2024年2月。经文献筛选、数据提取、质量评价后使用RevMan 5.4和Stata 17.0进行Meta分析。结果共纳入13篇文献,总样本量为1684例。其中,观察组采用老年综合护理,对照组采用常规护理。Meta分析结果显示,实施老年综合评估可有效降低老年高血压患者的偶测血压、24小时动态血压及血压变异性,差异具有统计学意义(P<0.05);老年综合评估还可有效提升高血压患者用药依从性,差异具有统计学意义(P<0.05);此外,相较于常规护理,老年综合评估可改善高血压患者的生活质量(P<0.05)。结论与常规护理相比,老年综合评估能明显降低高血压患者的血压,提升其用药依从性,一定程度上改善其生活质量。Objective To systematically assess the impact of comprehensive geriatric assessment(CGA)on elderly patients with hypertension.Methods A computerized search of The Cochrane Library,PubMed,Embase,Web of Science,China Biomedical Literature Database(CBM),VIP,China Knowledge Network(CNKI),and Wanfang databases for studies on the efficacy of a CGA of interventions for patients with hypertension was used,spanning from the inception of these databases to February 2024.Following literature review,data extraction,and quality checks,we conducted a Meta-analysis using RevMan 5.4 and Stata 17.0.Results Incorporating 13 studies,involving a cumulative sample of 1684 participants.Among them,the observation group adopted geriatric comprehensive care,and the control group adopted routine care.The results of Meta-analysis showed that the implementation of CGA could effectively reduce the occasional measured blood pressure,24-hour mobile blood pressure monitoring and fluctuations in blood pressure in elderly patients with hypertension(P<0.05);CGA boosts medication compliance for hypertension patients,and the difference was statistically significant(P<0.05).In addition,CGA interventions significantly enhanced the life quality among hypertensive patients,yielding statistically notable improvements(P<0.05).Conclusion The CGA outperforms usual care in lowering blood pressure for hypertensive,improving medication adherence,and enhancing quality of life.
