复方氨酚烷胺胶囊致维持性血液透析患者精神障碍的病例分析

摘　　要：肾脏是人体排泄药物及其代谢产物的重要器官。维持性血液透析患者肾脏排泄及代谢功能下降,可能出现药物代谢不充分,易引起药物毒性反应。复方氨酚烷胺胶囊是一种主要用于治疗感冒的甲类非处方用药,为复方制剂,组分有对乙酰氨基酚、盐酸金刚烷胺、马来酸氯苯那敏、人工牛黄、咖啡因,常见不良反应有偶见白细胞减少,厌食、恶心、呕吐、皮疹等,其中精神神经症状近年多见。我科2例终末期肾病维持性血液透析患者口服复方氨酚烷胺胶囊后出现幻视、幻听及共济失调等精神障碍,经高通量血液透析器进行血液透析治疗及停药后恢复正常。本文对2例病例进行讨论及文献分析,提示临床医师及终末期肾病维持性血液透析患者需警惕口服复方氨酚烷胺胶囊引发神经系统中毒反应。The kidney is an important organ for excreting drugs and their metabolites.In maintenance hemodialysis patients,renal excretion and metabolic function are decreased,which may lead to inadequate drug metabolism and an increased risk of drug toxicity.Compound paracetamol and amylamine capsule is a class A over-the-counter drug mainly used for the treatment of colds.This drug is a compound preparation containing acetaminophen,amantadine hydrochloride,chlorpheniramine maleate,artificial bezoar,and caffeine.Common adverse reactions include occasional leukopenia,anorexia,nausea,vomiting,and rash.In psychoneurotic symptoms have been reported more frequently.In this paper,we report two cases of maintenance hemodialysis with end-stage renal disease in our department who developed visual hallucinations,and ataxia after oral administration of compound paracetamol and amylamine capsules.These symptoms resolved after hemodialysis treatment with high-throughput hemodialyzer and discontinuation of the drug.This review of two cases,along with literature analysis,suggests that clinicians and maintenance hemodialysis patients with end-stage renal disease should be cautious of neurotoxic reactions caused by oral administration of compound paraminophen and amylamine capsules.
