摘　　要：目的:构建多学科慢性伤口延续护理"互联网+"信息平台,在医院-社区之间实现医疗信息共享,多层面给予患者个性化的干预措施并分析运行效果。方法:基于互联网医院信息技术搭建延续护理平台,于2022年6月至2023年6月,选取50例患者端用户和13位医护端用户共同使用平台,观察6个月后,通过纸质、电话回访与在线反馈方法收集分析平台的运行效果。结果:50例患者和13名医护人员应用体验总体评价为100%满意,患者自我管理行为能力得分(107.46±9.27)分,较应用前提升,差异有统计学意义(P<0.05)。结论:"互联网+"医院社区慢性伤口多学科延续护理信息平台,提高医护工作效率,同时为慢性伤口患者提供多种便捷问诊途径,提高患者的自我管理能力。Objective To build a multidisciplinary"Internet plus"information platform for chronic wound continuing care,realize medical information sharing between hospitals and communities,provide patients with personalized interventions at multiple levels,and analyze the operation effect.Methods A continuous nursing platform was built based on Internet hospital information technology.From June 2022 to June 2023,50 patient-side users and 13 medicalside users were selected to use the platform.After six months of observation,the operating effect of the platform was collected and analyzed through paper,telephone return visit and online feedback.Results The overall evaluation of the application experience of 50 patients and 13 medical staff was 100%satisfied.The score of self-management behavior ability of patients was(107.46±9.27)points,which was higher than that before the application,and the difference was statistically significant(P<0.05).Conclusions The"Internet plus"hospital community multidisciplinary continuous nursing information platform for chronic wounds can improve the efficiency of medical care work,provide multiple convenient ways for patients with chronic wounds to consult,and improve patients'self-management ability.
