摘  要:目的 探讨声触诊弹性成像(STE)联合c-Met、Galectin-3分子标志物检测在鉴别诊断甲状腺结节良恶性中的价值。方法 对93例甲状腺结节患者共125枚结节按照美国放射学会甲状腺影像报告和数据系统(ACR TI-RADS)进行分级,并应用STE测量结节内及周边2 mm厚度弹性硬度最大值E及E_(shell2.0);以病理为金标准,构建STE参数、分子标志物及ACR TI-RADS分级对结节诊断的受试者工作特征(ROC)曲线。结果 E_(shell2.0)组的曲线下面积(AUC)大于E值组及ACR TI-RADS分级组。E_(shell2.0)+c-Met组和E_(shell2.0)+c-Met+Galectin-3组的AUC均高于单独使用STE、c-Met和Galectin-3。结论 甲状腺结节E_(shell2.0)较结节内E值及ACR TI-RADS分级标准诊断效能更高,且联合c-Met或Galectin-3分子标志物有助于提高E_(shell2.0)的鉴别能力。Objective To investigate the value of sound touch elastography(STE)combined with c-Met and Ga-lectin-3 molecular markers in the differential diagnosis of benign and malignant thyroid nodules.Methods A total of 125 thyroid nodules from 93 patients were classified by ultrasound using the American College of Radiology thyroid imaging reporting and data system(ACR TI-RADS).The maximum elastic hardness value E and Eshel2.0 were meas-ured within and around the nodules,with a thickness of 2 mm by using STE technology;Taking pathological diagnosis as the gold standard,receiver operating characteristic(ROC)curves for the differential diagnosis of nodules were con-structed using STE parameters,molecular markers,and ACR TI-RADS grading.Results The area under the curve(AUC)of E_(shl2.0) group was greater than that of E-value group and ACR TI-RADS grading group.The AUC of E_(shel2.0)+c-Met group and E_(shel2.0)+c-Met+Galectin-3 group were higher than those of using STE,c-Met,and Galec-tin-3alone.Conclusions E_(sell2.0)is more effective in discerning benign or malignant nodules compared to the intra E-value and ACR TI-RADS grading standards.The combination of c-Met or Galectin-3 molecular markers can help improve E_(shel2.0's) diagnosis ability.
