摘 要:目的 探讨超声黏弹性成像技术评估桥本甲状腺炎(HT)患者和健康人群甲状腺黏弹性的可重复性和一致性。方法 连续性收集HT患者106例设为HT组,另选取同期健康志愿者53例设为健康组。两位不同经验的医师分别对HT组、健康组的甲状腺进行黏性值及弹性值检测。用组内和组间相关系数(ICC)评价测值的可重复性,使用Bland-Altman方法评价两位医师的一致性。结果 两位医师所测甲状腺黏性值、弹性值分别在HT组、健康组中比较,差异均无统计学意义(P>0.05)。两位医师各自测量的组内可重复性为0.81~0.93,测得HT组、健康组黏性值的组间ICC分别为0.89和0.89,弹性值的组间ICC分别为0.90和0.85。差异均无统计学意义(P>0.05)。结论 超声黏弹性成像技术检测HT患者和健康人群甲状腺黏弹性一致性和可重复性较好,几乎不受医师经验影响,为评估甲状腺病变提供全新的视角,值得临床推广应用。Objective To investigate the reproducibility and consistency of ultrasound viscoelastic imaging in e-valuating thyroid viscoelasticity in Hashimoto thyroiditis(HT)patients and healthy people.Methods A total of 106 patients with HT were selected as the HT group,and 53 healthy volunteers in the same period were selected as the healthy group.Two sonographers with different experience detected the viscosity value and elasticity value of thyroid gland in HT group and healthy group respectively.Intragroup and intergroup correlation coefficients(ICC)were used to evaluate the repeatability of the measured values,and Bland-Altman method was used to evaluate the consistency of the two physicians.Results There was no significant difference in thyroid viscosity and elasticity between HT group and healthy group(P>0.05).The intragroup repeatability measured by the two sonographers was 0.81-0.93,the intergroup ICC of viscosity value measured by HT group and healthy group were 0.89 and 0.89,and the intergroup ICC of elasticity value were 0.90 and 0.85,respectively.The differences were not statistically significant(P>0.05).Conclusions The viscoelasticity of thyroid in patients with HT and healthy people detected by ultrasound viscoelastic imaging is stable,reproducible,and almost unaffected by the experience of sonographers.It provides a new perspec-tive for the evaluation of thyroid lesions,and is worthy of clinical application.
