摘　　要：目的 探讨超声心动图图像前处理减影技术检测左冠状动脉内支架的可行性及价值。方法 对45例患者的52枚支架经图像前处理减影技术显示支架位置,测量大小;彩色多普勒显示支架血流情况,五彩混叠血流法判断支架狭窄程度;结果与手术记录及冠状动脉造影术(CAG)比较。结果 图像前处理减影技术检出40枚支架,可行性76.9%,测量支架长度小于手术记录,差异有统计学意义(P<0.001)。29枚支架显示血流信号,可行性72.5%;与CAG相比,支架狭窄程度判断的准确度为67.5%。结论 超声心动图图像前处理减影技术可检测左冠状动脉内支架,彩色多普勒五彩混叠血流法可判断支架内狭窄程度。Objective To explore the feasibility and value of transthoracic echocardiography pre-processing sub-traction technique in the detection of left coronary artery stents.Methods A total of 52 stents from 45 patients were examined by transthoracic echocardiography using two-dimensional ultrasound pre-processing subtraction technique,to observe the shape of coronary artery and its stent,to measure the length and diameter of the stent.Color Doppler flow and flow aliasing method were used to determine the degree of stenosis in the stent,and the results were compared with those of the coronary angiography(CAG)records.Results Transthoracic echocardiography detected 40 stents(3 in left main coronary artery,16 in proximal of anterior descending branch,14 in middle of anterior descending branch,5 in distal of anterior descending branch and 2 in distal of left circumflex branch)among 52 stents,and the de-tection rate was 76.9%.The length of the stent was measured at(22.6±4.3)mm and the diameter was about(2.5±0.5)mm,both were smaller than the procedural records(P<0.001).Compared with the results of CAG,echocardiography had a 67.5%accuracy in detection of the degree of stenosis in the stent.Conclusions Transthoracic echocardiography pre-processing subtraction technique can detect left coronary artery stents,with a smaller measure-ment of the length and diameter,but an accurate degree of stenosis.
