摘　　要：目的 探究剪切波弹性成像技术联合三维能量多普勒超声对冻胚移植患者妊娠结局预测的价值。方法 选取拟行冻胚移植的患者319例,根据其移植结局分为妊娠组(165例)及未妊娠组(154例),分析两组子宫内膜血流数据及杨氏模量值的差异性,绘制ROC曲线,分析其预测妊娠结局的准确度。结果 移植前1 d两组内膜厚度、容积、血管指数(VI)和血流指数(FI)均无明显差异(P>0.05);妊娠组血管血流指数(VFI)高于未妊娠组,平均杨氏模量值低于未妊娠组,差异有统计学意义(P<0.001)。VFI、平均杨氏模量值以及两者联合预测ROC曲线下面积分别为0.896、0.874、0.952。结论 剪切波弹性成像技术联合三维能量多普勒可准确预测妊娠结局,从而提高妊娠成功率。Objective To investigate the predictive value of shear wave elastography(SWE)and three-dimen-sional power Doppler ultrasound(3D-PDUS)in predicting pregnancy outcome in patients with frozen embryo transfer.Methods A total of 319 patients scheduled for frozen embryo transplantation were divided into the pregnant group(n=165 cases)and the non-pregnant group(n=154 cases)according to the transplantation outcomes.The differ-ences in endometrial blood flow and Young's modulus values were compared between the two groups.Receiver oper-ating characteristic(ROC)curve was drawn to analyze the accuracy in predicting pregnancy outcomes.Results There was no significant difference in endometrial thickness,endometrial volume,endometrial vascularity index(VI),and flow index(FI)between the two groups on the day before transplantation(P>0.05).However,the pregnancy group exhibited higher vascularization flow index(VFI)and lower average Young's modulus values,with statistical signifi-cance(P<o.001).The area under the curve(AUC)for VFI,average Young's modulus values,and their combina-tion were 0.896,0.874,and 0.952,respectively.Conclusions SWE combined with 3D-PDUS can accurately predict pregnancy outcomes in frozen embryo transfer,enhancing success rates.
