超声在急性踝扭伤致跗横关节损伤中的应用价值

摘　　要：目的 探讨肌骨超声在急性踝扭伤致跗横关节损伤(Chopart损伤)诊断中的价值。方法 回顾93例超声诊断为急性踝扭伤致Chopart损伤病例。对跟骰关节及距舟关节的损伤及合并伤情况进行分类。高频超声观察跟骰韧带、距舟韧带及趾短伸肌损伤后的超声表现。对韧带附着端撕脱骨折的情况结合性别、侧别进行分析,并与X线检查结果进行比对。结果 Chopart损伤结合合并伤情况可分为4种类型。单纯Chopart损伤与有合并伤的Chopart损伤在性别构成比上差异有统计学意义(P<0.05)。跟骰韧带与距舟韧带附着端伴有或不伴有撕脱骨折两种情况的构成比差异有统计学意义(P<0.05)。Chopart损伤不仅累及韧带,也容易导致趾短伸肌损伤。与X线检查相比,超声对于跟骰韧带及距舟韧带附着端撕脱骨折的诊断更有价值。结论 超声是一种简便有效的方法,可用于急性踝扭伤所致Chopart损伤的检查及评估。Objective To evaluate the value of musculoskeletal ultrasonography in diagnosis of transverse tarsal joint injury(Chopart injury)caused by acute ankle sprain.Methods A total of 93 cases of Chopart injury caused by acute ankle sprain by ultrasound were analyzed retrospectively.The cases were classified according to the injuries and associated injuries of calcaneoscinate and talonavoid joint.High-frequency ultrasonography was used to observe the ultrasonographic appearance of the injury of calcaneocuboid ligament,talonavicular ligament and extensor digitorum brevis.The avulsion fractures of the attached end of ligament were analyzed by sex and side,and compared with X-ray results.Results Chopart injury and combined injury could be divided into 4 types.There was significant difference in the sex component ratio between Chopart injury and Chopart injury with combined injury(P<0.05).There was sig-nificant difference in the proportion of ligments with and without avulsion fracture between calcaneocuboid and talona-vicular ligament(P<0.05).Chopart injury not only involved ligaments,but also caused extensor digitorum brevis in-jury.Compared with X-ray examination,ultrasonography was more valuable in the diagnosis of avulsion fractures of calcaneocuboid and talonavicular ligament.Conclusions Ultrasonography is a simple and effective method,which can be used to evaluate Chopart injury caused by acute ankle sprain.
