早孕期胎儿下腔静脉离断的产前超声诊断方法

摘　　要：目的 探讨早孕期胎儿下腔静脉离断(IIVC)的产前超声诊断方法。方法 选取产前超声诊断的10例胎儿IIVC,分析其早孕期超声图像,并与同时期正常组50例胎儿进行对照,随访中孕期超声情况或生后、引产后结果,总结早孕期超声征象。结果 10例IIVC胎儿均在早孕期诊断,其中有8例三血管切面彩色多普勒血流成像显示了奇静脉,与正常组胎儿差异有统计学意义(P<0.05)。10例中左侧异构8例,孤立IIVC 1例,1例合并左心发育不良综合征;8例左侧异构中有4例出现心动过缓,4例合并内脏位置异常;8例中有3例合并了严重的心内结构异常。10例中3例出生,7例引产。结论 早孕期三血管切面彩色多普勒血流成像中奇静脉显示是提示IIVC的有效线索,合并心脏节律异常时无论是否伴有内脏位置异常均需警惕左侧异构的存在。Objective To explore the prenatal ultrasound diagnostic method for fetal interrupted inferior vena ca-va(IIVC)in first trimester.Methods A total of 10 cases of fetal IIVC diagnosed by prenatal ultrasound were select-ed.Their early pregnancy ultrasound images were analyzed and compared with 50 normal fetuses in the same period.The follow-up on mid pregnancy ultrasound conditions or postnatal and induced labor results were conducted to sum-marize first trimester ultrasound signs.Results All 10 fetuses with IIVC were diagnosed in early pregnancy,and 8 of them showed anomalous azygos vein on color Doppler blood flow imaging in the three vessel view,which was signifi-cantly different from the normal group of fetuses(P<o.05).Among the 10 cases,there were 8 cases of left isomer-ism,1 case of isolated IIvC,and 1 case of hypoplastic left heart syndrome;Among the 8 cases of left isomerism,4 cases showed bradycardia and 4 cases were combined with abnormal visceral position;Among the 8 cases,3 cases had severe intracardiac structural abnormalities.Out of 1o cases,3 were born and 7 were induced.Conclusions In first trimester,display anomalous azygos vein in three vessel view with color Doppler flow imaging is an effective clue to di-agnosis IIVC.When combined with abnormal cardiac rhythm,whether accompanied by visceral position abnormalities or not,it is necessary to be alert to the presence of left isomerism.
