血清sTREM2与老年急性冠脉综合征的相关性研究

摘　　要：目的探讨血清中可溶性髓系细胞触发受体(soluble triggering receptor expressed on myeloid cells,sTREM)2与老年急性冠脉综合征(acute coronary syndromes,ACS)的相关性。方法选取2024年3月至8月因胸痛于石河子大学第一附属医院住院并行冠状动脉造影的60岁以上患者165例作为研究对象,根据患病情况分为急性心肌梗死组(n=56)、不稳定性心绞痛组(n=68)、非冠状动脉粥样硬化性心脏病(简称非冠心病)组(n=41),比较三组研究对象的临床资料,采用Logistic回归分析筛选出独立危险因素,采用受试者操作特征曲线(receiver operating characteristic curves,ROC曲线)评价血清sTREM2对老年ACS的预测价值。结果非冠心病组、不稳定性心绞痛组、急性心肌梗死组患者的血清sTREM2水平依次升高,组间差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示血清sTREM2水平升高是不稳定性心绞痛和急性心肌梗死的独立危险因素(P<0.05),且与急性心肌梗死的关联更强。ROC曲线分析结果显示,血清sTREM2对老年ACS有预测价值,敏感度为66.9%,特异性为78.0%,曲线下面积为0.771。结论血清sTREM2可预测老年ACS,其水平可能与老年ACS患者的冠状动脉病变程度相关。Objective To investigate the association between serum soluble triggering receptor expressed on myeloid cells 2(sTREM2)level and acute coronary syndromes(ACS)in the elderly.Methods A total of 165 patients over 60 years old who underwent coronary angiography in the First Hospital Shihezi University from March to August 2024 were included and divided into acute myocardial infarction group(n=56),unstable angina pectoris group(n=68),and non-coronary heart disease(CHD)group(n=41).Clinical data of three groups were analyzed and compared,Logistic regression analysis was used to screen out independent risk factors,and receiver operating characteristic(ROC)curve was used to evaluate predictive value of serum sTREM2 for ACS in elderly ACS patients.Results Serum sTREM2 levels of patients in non-CHD group,unstable angina pectoris group,and acute myocardial infarction group were sequentially elevated,and differences between groups were statistically significant(P<0.05).Logistic regression analysis showed that elevated serum sTREM2 level was an independent risk factor for acute myocardial infarction and unstable angina pectoris(P<0.05),and was more closely related to acute myocardial infarction.ROC curve analysis showed that serum sTREM2 had a predictive value in elderly ACS patients,with sensitivity of 66.9%,specificity of 78.0%,and area under the curve of 0.771.Conclusion Serum sTREM2 can predict ACS in elderly ACS patients,and its levels may correlate with the degree of coronary artery lesions in elderly ACS patients.
