摘  要:目的 总结磁共振3D-T_(2)WI-DESS和扩散张量成像(DTI)序列在健康志愿者腰骶神经形态及功能学的特点,为这两项技术在腰骶神经病变患者中的应用提供解剖学依据。方法 纳入96名健康志愿者,年龄18~79岁,分为青年组(18~44岁)、中年组(45~59岁)和老年组(>60岁),所有备试者均在西门子Prisma 3.0 T MRI设备上检查,扫描序列包括3D-T_(2)WI-DESS和DTI,采集数据使用西门子Syngo.via后处理工作站处理。由两名医师独立测量腰骶丛L_(3)~S_(1)双侧神经根节前纤维和节后纤维2个感兴趣区(ROI)的DTI参数值,计算其平均值。采用两独立样本t检验比较所有入组患者同一节段双侧神经根穿出角度及神经纤维各向异性分数(FA)值的差异;采用单因素方差分析分别比较L_(3)~S_(1)神经根穿出角度及FA值的差异;采用单因素方差分析分别比较不同年龄组L_(3)~S_(1)神经夹角及FA值的差异。结果 L_(3)~S_(1)节段双侧节前及节后神经纤维FA值差异均有统计学意义(P<0.05);不同年龄段腰骶不同节段的神经根FA值稍有差异,左侧L_(5)节后神经纤维(P<0.01)、双侧S_(1)节前及S_(1)节后神经纤维差异有统计学意义(P<0.05);左、右侧L_(3)~S_(1)神经根出硬膜囊角度差异均有统计学意义(P<0.01);不同年龄左侧L_(3)和右侧S_(1)神经根出硬膜囊角度差异有统计学意义(P<0.05)。结论 不同节段及不同年龄组腰骶神经根形态及功能学均存在一定的差异;3D-T_(2)WI-DESS序列结合DTI可以分别从结构及功能成像角度提供人体腰骶神经的影像解剖学定量信息。Objective To summarize the morphological and functional characteristics of the lumbosacral nerves in healthy volunteers using MRI 3D-T2WI-DESS and DTI sequences,providing an anatomical basis for the application of these two techniques in patients with lumbosacral nerve lesions.Methods Ninety-six healthy volunteers aged 18 to 79 were included and divided into three groups:young group(18-44 years),middle-aged group(45-59 years),and elderly group(>60 years).All participants underwent examinations on a Siemens Prisma 3.0 T MRI system,with scanning sequences including 3D-T_2WI-DESS and DTI.Data acquisition was processed using the Siemens Syngo.via workstation.Two physicians independently measured the DTI parameter values of the anterior and posterior nerve fibers of the bilateral L_(3)-S_(1) nerve roots,and calculated the average values.Independent sample t-tests were used to compare differences in the nerve root exit angles and nerve fiber FA values between both sides of the same segment in all enrolled patients.One-way ANOVA was used to compare differences in the exit angles and FA values of L_(3)-S_(1) nerve roots.One-way ANOVA was also used to compare differences in nerve root angles and FA values among different age groups.Results There were statistically significant differences in FA values of bilateral anterior and posterior nerve fibers at the L_(3)-S_(1) segments(P<0.05).Minor differences in FA values of nerve roots at different lumbosacral segments were observed across different age groups,with statistical significance seen in the left L_(5) posterior nerve fiber(P<0.01) and bilateral S_(1) anterior and posterior nerve fibers(P<0.05).Statistically significant differences were found in the angles of nerve root exit from the dural sac at L_(3)-S_(1) on both sides(P<0.01),as well as in the angles of the left L_(3) and right S_(1) nerve roots among different age groups(P<0.05).Conclusion Variations in morphology and functional characteristics of lumbosacral nerve roots exist across different segments and age gro
