摘  要:目的探讨胎球蛋白A(AHSG)和铁蛋白(SF)表达与冠心病患者冠状动脉慢血流的相关性。方法回顾性分析2020年1月至2023年2月邢台医学高等专科学校第二附属医院收治的120例冠心病患者,均行冠状动脉造影,依据心肌梗死溶栓治疗(TIMI)帧计数法,TIMI帧计数>20帧为冠状动脉慢血流,将患者分成冠状动脉慢血流组和正常组,检测两组患者入院时血清AHSG和SF,对血清AHSG和SF与TIMI帧计数进行相关性分析,多因素Logistic回归分析确定冠状动脉慢血流的影响因素,ROC曲线分析血清AHSG和SF对冠状动脉慢血流的预测价值。结果冠状动脉慢血流组血清AHSG低于正常组,SF高于正常组,差异有统计学意义(t=16.373、8.825,P<0.01)。冠状动脉慢血流组TIMI帧计数为(27.31±3.04)帧,正常组TIMI帧计数为(10.25±1.54)帧,血清AHSG与TIMI帧计数呈负相关(r=-0.762,P<0.01),血清SF与TIMI帧计数呈正相关(r=0.748,P<0.01)。Logistic回归分析结果表明,AHSG和SF是冠状动脉慢血流的影响因素(OR=0.554、2.184,P<0.05)。ROC曲线显示,血清AHSG临界值为443.26 mg/L,其对应的敏感度为78.05%,特异度为81.01%,AUC为0.771;SF临界值为192.32 ng/mL,其对应的敏感度为73.17%,特异度为74.68%,AUC为0.875;两者联合诊断在最佳临界值时的敏感度为95.12%,特异度为94.94%,AUC为0.929。结论血清AHSG和SF在冠心病冠状动脉慢血流患者中异常表达,是冠心病患者冠状动脉慢血流的影响因素,并可用于预测冠状动脉慢血流发生。Objective To study the correlation between the expression of alpha-2-heremansschmid glycoprotein(AHSG)and serum ferritin(SF)and coronary slow flow in patients with coronary heart disease.Methods A retrospective analysis on 120 patients with coronary heart disease admitted to the Second Affiliated Hospital of Xingtai Medical College from January 2020 to February 2023 was performed.Coronary angiography was performed,and patients were classified into the coronary slow flow group and the control group based on the thrombolysis in myocardial infarction(TIMI)frame count method,with TIMI frame count>20 frames indicating coronary slow flow.Serum levels of AHSG and SF of the two groups were mearsured at admission.A correlation analysis of the relationship between serum AHSG and SF with TIMI frame count was conducted.Multivariate Logistic regression analysis was used to determine factors influencing coronary slow flow.Receiver operating characteristic(ROC)curve analysis was performed to evaluate the predictive value of serum AHSG and SF for coronary slow flow.Results The coronary slow flow group had significantly lower serum AHSG and higher SF levels compared to the control group(t=16.373,8.825;P<0.01).The TIMI frame count was(27.31±3.04)frames in the coronary slow flow group and(10.25±1.54)frames in the control group.Serum AHSG was negatively correlated with TIMI frame count(r=-0.762,P<0.01),while serum SF was positively correlated with TIMI frame count(r=0.748,P<0.01).Logistic regression analysis indicated that AHSG and SF were factors influencing coronary slow flow(OR=0.554,2.184;P<0.05).ROC curve analysis showed that the critical value for serum AHSG was 443.26 mg/L,with a sensitivity of 78.05%,specificity of 81.01%,and AUC of 0.771.For SF,the critical value was 192.32 ng/mL,with a sensitivity of 73.17%,specificity of 74.68%,and AUC of 0.875.The sensitivity,specificity and AUC of the combined diagnosis were 95.12%,94.94%and 0.929 at the optimal critical value.Conclusion The abnormal expressions of serum AHSG and SF a
