磁电联合肌筋膜手法治疗产后非松弛性盆底功能障碍的临床效果

摘　　要：目的:探讨盆底磁电联合肌筋膜手法治疗产后非松弛性盆底功能障碍性疾病的效果。方法:回顾性分析2021年3月至2024年3月南京市鼓楼区阅江楼社区卫生服务中心妇保科收治的565例产后非松弛性盆底功能障碍性疾病患者,所有患者均接受腹式呼吸训练+肌筋膜手法治疗,其中电刺激组315例给予盆底生物反馈电刺激,磁电组250例实施盆底磁电联合治疗。两组治疗前后采用视觉模拟量表(VAS)评估盆底肌筋膜疼痛程度,Glazer盆底表面肌电评估盆底肌肉功能。结果:与治疗前比较,治疗后两组VAS评分均降低,前静息电位、后静息电位平均值、Ⅰ、Ⅱ类纤维各类指标均有改善,差异有统计学意义(P<0.05)。治疗后两组比较,磁电组闭孔内肌疼痛改善优于电刺激组,后静息电位平均值低于电刺激组,差异有统计学意义(P<0.05)。结论:磁电联合综合治疗和电刺激治疗均可改善非松弛性盆底功能障碍,磁电联合综合治疗对降低后静息电位、减轻肌筋膜疼痛效果优于生物反馈电刺激治疗,值得推广。Objective:To investigate the effect of pelvic floor magnetoelectric combined with myofascial release in the treatment of postpartum nonrelaxing pelvic floor dysfunction.Methods:565 patients with postpartum nonrelaxing pelvic floor dysfunction treated in the women's health care department of Yuejianglou community from March 2021 to March 2024 were reviewed.All patients received abdominal breathing training and myofascia release.315 patients in the electrical group received pelvic floor biofeedback electrical stimulation,and 250 patients in the magnetoelectric group received pelvic floor magnetoelectric combined therapy.Visual analogue scale(VAS)was used to evaluate pelvic floor myofascial pain before and after treatment,and electromyographic evaluation was used to evaluate pelvic floor muscle function.Results:Compared with before treatment,VAS scores of both groups were decreased after treatment,and various indexes of pre-resting potential,post-resting potential,classⅠandⅡfibers were improved,with statistical significance(P<0.05).Compared with the two groups after treatment,the pain relief of internal obturator muscle in the magnetoelectric group was better than that in the electrical group,and the mean value of post-resting potential was lower than that in the electrical group,the difference was statistically significant(P<0.05).Conclusion:Both magnetoelectric and electrical stimulation therapy can improve nonrelaxing pelvic floor dysfunction,and magnetoelectric therapy is better than biofeedback electrical stimulation therapy in reducing post-resting potential and relieving myofascial pain,which is worthy of promotion.
