参麦注射液对西藏高原高血红蛋白人群悬浮红细胞保存质量的影响

摘  要:目的 考察参麦注射液对西藏高原高血红蛋白人群悬浮红细胞的保存质量的影响。方法 将西藏自治区血液中心采集的400 mL全血(共8袋),3 000 g离心10 min后去除大部分血浆,再加入100 mL MAP保存液得到高原悬浮红细胞,然后将其均分为3份。一份为对照组,另一份加入15 mL MAP保存液,为加量组。第3份,将参麦注射液用MAP稀释后,再将15 mL添加了参麦的MAP保存液加入到红细胞中,参麦注射液的终浓度为1%,为参麦组。于d1、d21、d35分别检测血常规、pH值、电解质、葡萄糖、乳酸、游离Hb、三磷酸腺苷(ATP)、P50、磷脂酰丝氨酸(PS)等指标。结果 加量组和参麦组的Hb浓度、Hct都明显低于对照组,分别为(179.3±17.8)vs(181.0±17.1)vs(199.1±19.5)g/L,(53.2±2.6)%vs(53.3±2.5)%vs(58.4±3.1)%,至保存末期,3组的数据仍保持这一规律。保存中末期,加量组和参麦组的葡萄糖、Na^(+)含量高于对照组,乳酸和K^(+)含量低于对照组;但保存末期,参麦组的葡萄糖、Na^(+)含量高于加量组,乳酸和K^(+)含量低于加量组。在保存的d1~d35,参麦组的溶血率明显低于对照组和加量组。在保存的d21和d35,参麦组的PS表达率明显低于对照组和加量组,分别为(6.52±0.40)%vs(7.24±0.91)%vs(8.27±0.93)%,(7.29±0.53)%vs(9.37±0.82)%vs(8.39±0.76)%。结论 添加参麦注射液制备的西藏高海拔的悬浮红细胞的溶血率和PS都明显低于对照组和加量组,有利于减少溶血和减缓红细胞的老化,对西藏高原人群悬浮红细胞的保存质量,有一定的改善作用。Objective To investigate the effect of Shenmai injection on the preservation quality of suspended red blood cells of high hemoglobin population in Xizang plateau.Methods Whole blood(400 mL,n=8)collected by the Xizang Autonomous Region Blood Center was centrifuged at 3000 g for 10 minutes to remove most of the plasma,followed by the addition of 100 mL of MAP preservation solution to obtain plateau suspended red blood cells,which were then divided into three equal portions.One portion was the control group,and another part had 15 mL of MAP preservation solution added,which was the dosage group.The third portion involved diluting Shenmai injection with MAP,followed by addition of 15 mL of MAP preservation solution containing Shenmai to the red blood cells,resulting in a final concentration of Shenmai injec-tion of 1%,which was the Shenmai group.Blood routine,pH value,electrolytes,glucose,lactate,free Hb,adenosine triphosphate(ATP),P50,phosphatidylserine(PS)and other indicators were detected at day 1,21 and 35,respectively.Results The Hb concentration and Hct of the dosage group and the Shenmai group were significantly lower than those of the control group,with values of(179.3±17.8)vs(181.0±17.1)vs(199.1±19.5)g/L for Hb concentration and(53.2±2.6)%vs(53.3±2.5)%vs(58.4±3.1)%for Hct.The three groups maintained this pattern until the end of storage.In the middle and late stages of preservation,the glucose and Na+contents in the dosage group and the Shenmai group were higher than those in the control group,while the lactate and K+contents were lower than those in the control group.At the end of storage,the glucose and Na+content of the Shenmai group was higher than that of the dosage group,while the lactate and K+content were lower than that of the dosage group.From day 1 to day 35 of storage,the hemolysis rate of the Shenmai group was significantly lower than that of the control group and the dosage group.On day 21 and 35 of storage,the PS ex-pression rate in the Shenmai group was significantly lower than that in
