不同年资手术医师行腹腔镜下胆囊切除术结果分析

摘　　要：目的分析不同年资手术医师对胆囊相关疾病行腹腔镜下胆囊切除术结果的差异。方法选取174例行腹腔镜下胆囊切除术的胆囊相关疾病患者,根据术者不同分为高年资医师组(主任医师或副主任医师,91例)、低年资医师组(工作不满3年的主治医师,83例)。比较两组患者的住院日、手术时间、麻醉时间、术后并发症发生率、住院费用、出院30 d内再入院率。结果高年资医师组患者手术时间(22.35±6.47)min、麻醉时间(47.28±10.83)min短于低年资医师组的(32.73±7.64)、(56.76±12.26)min,住院费用(10896.49±489.73)元少于低年资医师组的(11398.61±516.49)元,差异有统计学意义(P<0.05)。两组患者的住院日、术后并发症发生率、出院30 d内再入院率对比差异无统计学意义(P>0.05)。两组均出现2例并发症患者,对症处理好转出院;两组均有1例30 d内再次住院患者,治疗好转出院。结论对于相同诊断及相同手术方式患者,术者年资不同,住院期间的医疗资源消耗会产生较大差异。Objective To analyze the difference in the results of laparoscopic cholecystectomy for gallbladder-related diseases by surgeons with different seniorities.Methods 174 cases of gallbladder-related diseases patients who underwent laparoscopic cholecystectomy were divided into high seniority group(chief physician or deputy chief physician,91 cases)and low seniority group(attending physicians who have worked for less than 3 years,83 cases).Comparison of length of hospital stay,operation time,anesthesia time,incidence of postoperative complications,hospitalization expenses,and readmission rate within 30 d after discharge between the two groups.Results The operation time and anesthesia time were(22.35±6.47)and(47.28±10.83)min in the high seniority group,which were shorter than(32.73±7.64)and(56.76±12.26)min in the low seniority group;the high seniority group had a lower hospitalization cost of(10896.49±489.73)yuan than(11398.61±516.49)yuan in the low seniority group;the difference was statistically significant(P<0.05).There was no statistically significant difference in the length of hospital stay,incidence of postoperative complications,and readmission rate within 30 d after discharge between the two groups(P>0.05).Both groups had 2 patients with complications,who were treated symptomatically and improved before being discharged;both groups had 1 patient who was readmitted within 30 d and was discharged after improvement of treatment.Conclusion Regarding patients who receive the same diagnosis and operation,the consumption of their medical resources during hospitalization will be significantly different due to the difference in the seniority of surgeons.
