摘 要:目的探究中医定向透药疗法联合机械辅助排痰在小儿大叶性肺炎治疗中的效果。方法60例大叶性肺炎患儿,根据治疗方式的差异性进行分组,即对照组和研究组,各30例。对照组患儿予以机械辅助排痰治疗,研究组患儿采取中医定向透药疗法联合机械辅助排痰治疗。对比两组患儿的临床症状(发热、咳嗽咳痰、肺部啰音、气促)消失时间,免疫指标[免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)],肺功能指标[最大肺活量(VCmax)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、呼气峰值流速(PEF)],临床疗效。结果研究组患儿的发热、咳嗽咳痰、肺部啰音、气促消失时间分别为(6.58±2.37)、(5.75±1.32)、(3.56±0.78)、(1.75±0.58)d,均较对照组的(11.96±2.45)、(8.59±1.24)、(5.91±0.42)、(3.42±0.93)d短(P<0.05)。治疗后,两组患儿的IgG、IgA、IgM均较治疗前降低,且研究组的IgG(8.95±0.18)g/L、IgA(0.99±0.38)g/L、IgM(1.02±0.46)g/L低于对照组的(10.64±0.75)、(1.18±0.35)、(1.31±0.44)g/L(P<0.05)。治疗后,两组患儿的VCmax、FVC、FEV1、PEF均较治疗前升高,且研究组的VCmax(2.95±0.18)L、FVC(3.08±0.38)L、FEV1(2.91±0.44)L、PEF(3.77±0.61)L/s高于对照组的(2.43±0.25)L、(2.72±0.25)L、(2.62±0.46)L、(2.52±0.68)L/s(P<0.05)。治疗后,研究组的治疗总有效率为96.67%,对照组为76.67%,研究组高于对照组(χ^(2)=5.192,P=0.023<0.05)。结论将中医定向透药疗法联合机械辅助排痰运用于小儿大叶性肺炎治疗中可有效缓解患儿咳嗽、咳痰等临床症状,改善肺部功能,提高临床疗效,值得临床推广应用。Objective To explore the effect of traditional Chinese medicine targeted drug penetration therapy combined with mechanical auxiliary expectoration in the treatment of lobar pneumonia in children.Methods 60 children with lobar pneumonia were divided into control group and study group according to the difference in treatment methods,with 30 cases in each group.The control group was treated with mechanical auxiliary expectoration,while the study group was treated with traditional Chinese medicine targeted drug penetration combined with mechanical auxiliary expectoration.Both groups were compared in terms of disappearance time of clinical symptoms(fever,cough and sputum,pulmonary rales,shortness of breath),immune indicators[immunoglobulin G(IgG),immunoglobulin A(IgA),immunoglobulin M(IgM)],pulmonary function indicators[maximum vital capacity(VCmax),forced vital capacity(FVC),forced expiratory volume in one second(FEV1),peak expiratory flow rate(PEF)]and clinical efficacy.Results The fever reduction time,disappearance time of cough and sputum,pulmonary rale and shortness of breath were(6.58±2.37),(5.75±1.32),(3.56±0.78)and(1.75±0.58)d in the study group,which were shorter than(11.96±2.45),(8.59±1.24),(5.91±0.42)and(3.42±0.93)d in the control group(P<0.05).After treatment,the levels of IgG,IgA and IgM in both groups were lower than those before treatment;the study group had IgG of(8.95±0.18)g/L,IgA of(0.99±0.38)g/L and IgM of(1.02±0.46)g/L,which were lower than(10.64±0.75),(1.18±0.35)and(1.31±0.44)g/L in the control group(P<0.05).After treatment,VCmax,FVC,FEV1 and PEF in both groups were higher than those before treatment;the study group had VCmax of(2.95±0.18)L,FVC of(3.08±0.38)L,FEV1 of(2.91±0.44)L and PEF of(3.77±0.61)L/s,which were higher than(2.43±0.25)L,(2.72±0.25)L,(2.62±0.46)L and(2.52±0.68)L/s in the control group(P<0.05).After treatment,the total effective rate was 96.67%in the study group and 76.67%in the control group;the study group was higher than the control group(χ^(2)=5.192,P=0.0
