独活寄生汤内服结合熏洗治疗膝骨关节炎风寒湿痹证患者的疗效及对关节肿胀的影响

摘　　要：目的观察独活寄生汤内服结合熏洗治疗膝骨关节炎风寒湿痹证患者的疗效及对关节肿胀的影响。方法130例膝骨关节炎风寒湿痹证患者作为观察对象,依据平行对照设计原则分为对照组和研究组,各65例。对照组患者给予双氯芬酸钠缓释片口服治疗,研究组患者在对照组的基础上给予独活寄生汤内服结合熏洗治疗。比较两组临床疗效、中医证候积分、骨关节炎严重程度、关节功能障碍情况及生活质量。结果研究组患者临床治疗总有效率为90.77%,同对照组的76.92%比较明显更高(P<0.05)。治疗后,两组患者膝关节疼痛、肿胀、压痛、晨僵、屈伸不利及功能障碍积分同治疗前比较均明显降低,且研究组患者膝关节疼痛、肿胀、压痛、晨僵、屈伸不利及功能障碍积分同对照组比较明显更低(P<0.05)。治疗后,两组患者西安大略和麦克马斯特大学骨关节炎调查量表(WOMAC)评分、奎森功能障碍指数(Lequesne)评分及关节炎生活质量测量量表2(AIMS2)评分同治疗前比较均显著改善,且研究组患者WOMAC评分(62.97±15.85)分及Lequesne评分(3.72±0.64)分同对照组的(82.03±15.03)、(3.72±0.64)分比较明显更低,AIMS2评分(179.53±24.61)分同对照组的(143.24±23.34)分比较明显更高(P<0.05)。结论采用独活寄生汤内服结合熏洗对膝骨关节炎风寒湿痹证患者进行治疗,能够显著提高患者临床疗效,改善患者关节疼痛、肿胀等症状,减轻患者疾病严重程度,改善患者关节功能障碍及生活质量。Objective To observe the curative effect of Duhuo Jizhi Decoction combined with fumigation in the treatment of knee osteoarthritis patients with wind-cold-dampness arthralgia syndrome and its influence on joint swelling.Methods 130 cases of knee osteoarthritis patients with wind-cold-dampness arthralgia syndrome as study subjects were divided into control group and study group according to the principle of parallel control principle,each with 65 cases.The control group was given diclofenac sodium sustained-release tablet orally,and the study group was given Duhuo Jizhi Decoction combined with fumigation treatment on the basis of the control group.The clinical efficacy,traditional Chinese medicine syndrome score,severity of osteoarthritis,joint dysfunction and quality of life of the two groups were compared.Results The total clinical effective rate of 90.77%in the study group was significantly higher than 76.92%in the control group(P<0.05).After treatment,the scores of knee pain,swelling,tenderness,morning stiffness,inhibited bending and stretching and dysfunction in both groups were significantly lower than those before treatment,and the scores of knee pain,swelling,tenderness,morning stiffness,inhibited bending and stretching and dysfunction in the study group were significantly lower than those in the control group(P<0.05).After treatment,the Western Ontario and McMaster University Osteoarthritis Index(WOMAC)score,Lequesne score and Arthritis Impact Measurement Scales 2(AIMS2)score in both groups were significantly improved compared with before treatment;the study group had WOMAC score of(62.97±15.85)points and Lequesne score of(3.72±0.64)points,which were significantly lower than(82.03±15.03)and(3.72±0.64)points in the control group;the study group had a significantly higher AIMS2 score of(179.53±24.61)points than(143.24±23.34)points in the control group(P<0.05).Conclusion The use of Duhuo Jizhi Decoction combined with fumigation in the treatment of knee osteoarthritis patients with wind-cold-dampness ar
