摘 要:目的分析手汗症患者胸腔镜下胸交感神经切断术(ETS)后发生重度代偿性多汗(CH)的影响因素。方法选取2018年1月—2023年1月安徽医科大学第二附属医院收治的行ETS治疗的手汗症患者。使用电子调查问卷和患者病历收集患者临床资料,采用多因素Logistic回归分析进行影响因素分析。结果本调查共发放调查问卷221份,回收有效问卷209份,回收有效率为94.57%。209例研究对象中,177例术后发生CH。术后1个月与3个月重度CH发生率比较,差异有统计学意义(P<0.05)。术后12个月,在177例术后发生CH患者中,33例发生重度CH,纳入重度CH组;144例发生轻中度CH,纳入非重度CH组。单因素分析结果显示,重度CH组与非重度CH组年龄、体重指数、合并颅面多汗和术中交感神经切断部位比较,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,术中交感神经切断部位是手汗症患者ETS后发生重度CH的影响因素(P<0.05)。结论术中交感神经切断部位是手汗症患者ETS后发生重度CH的影响因素。Objective To analyze the influencing factors of severe compensatory hyperhidrosis(CH)after endoscopic thoracic sympathectomy(ETS)in patients with palmar hyperhidrosis.Methods Patients with palmar hyperhidrosis treated with ETS in the Second Affiliated Hospital of Anhui Medical University from January 2018 to January 2023 were selected.Clinical data of patients were collected by electronic questionnaire and patient medical records,and the influencing factors were analyzed by multivariate Logistic regression analysis.Results A total of 221 questionnaires were distributed in this survey,and 209 valid questionnaires were recovered,with an effective recovery rate of 94.57%.A total of 177 patients developed CH after surgery among the 209 subjects.There was significant difference in the incidence rate of severe CH between after operation for 1 month and 3 months(P<0.05).Among the 177 patients with postoperative CH after operation for 12 months,33 patients with severe CH were included in the severe CH group,while 144 patients with mild to moderate CH were included in the non-severe CH group.Univariate analysis showed that there were significant differences in age,body mass index,combined craniofacial hyperhidrosis and intraoperative sympathetic nerve transection site between the severe CH group and the non-severe CH group(P<0.05).Multivariate Logistic regression analysis showed that the nerve transection site of sympathectomy during operation was the influencing factor of severe CH after ETS in patients with palmar hyperhidrosis(P<0.05).Conclusions The nerve transection site of sympathectomy during operation is the influencing factor of severe CH after ETS in patients with palmar hyperhidrosis.
