显微镜下CO_(2)激光鼓膜打孔术与传统鼓膜切开加置管术治疗分泌性中耳炎的临床效果比较

摘　　要：目的比较显微镜下CO_(2)激光鼓膜打孔术与传统鼓膜切开加置管术治疗分泌性中耳炎的临床效果。方法选取2023年9月—2024年3月在广元市第一人民医院耳鼻咽喉科就诊的分泌性中耳炎患者40例,根据治疗术式不同,分为观察组与对照组,各20例。对照组采用传统鼓膜切开加置管术,观察组采用显微镜下CO_(2)激光鼓膜打孔术,比较两组治疗效果、纯音听阈、生活质量[阻塞性睡眠呼吸暂停疾病特异性生活质量调查量表(OSA-18)评分]及并发症。结果治疗3个月后,观察组总有效率高于对照组,纯音听阈和OSA-18评分低于对照组;治疗期间,观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论相较于传统鼓膜切开加置管术,采用显微镜下CO_(2)激光鼓膜打孔术更能改善分泌性中耳炎患者纯音听阈,提升治疗效果,改善生活质量,降低相关并发症发生风险。Objective To compare the clinical effect of microscopic CO_(2)laser myringotomy and traditional myringotomy with tube insertion in the treatment of secretory otitis media.Methods A total of 40 patients with secretory otitis media who visited the Department of Otolaryngology at the First People's Hospital of Guangyuan from September 2023 to March 2024 were selected and divided into an observation group and a control group,with 20 cases in each group.The control group underwent traditional myringotomy with tube insertion,while the observation group underwent microscopic CO_(2)laser myringotomy.The efficacy,puretone hearing threshold,quality of life[Obstructive Sleep Apnea Disease-Specific Quality of Life Survey Instrument-18(OSA-18)score],and complications were compared between the two groups.Results After treatment,the total effective rate in the observation group were higher than those in the control group,and the pure-tone hearing threshold and OSA-18 score was lower than that in the control group;during the treatment period,the total incidence of complications in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusions Compared with traditional myringotomy with tube insertion,microscopic CO_(2)laser myringotomy can better improve the pure-tone hearing threshold of patients with secretory otitis media,enhance treatment efficacy,improve quality of life,and reduce the risk of related complications.
