腹腔镜疝修补术与开放式无张力疝修补术治疗成人腹股沟疝的效果比较

摘　　要：目的比较腹腔镜疝修补术与开放式无张力疝修补术治疗成人腹股沟疝的效果。方法选取2023年1月—2024年3月济南市莱芜区口镇街道社区卫生服务中心收治的58例成人腹股沟疝患者,采用随机数字表法分为观察组和对照组,各29例。观察组患者接受腹腔镜疝修补术,对照组患者接受开放式无张力疝修补术。比较两组围术期指标、手术效果、疼痛程度[视觉模拟评分法(Visual Analogue Scale,VAS)]、并发症和复发情况。结果观察组术中出血量少于对照组,术后排气时间、首次下床时间及住院时间短于对照组;术后12、24、72 h,观察组VAS评分均低于对照组;观察组术后并发症发生率及随访3个月复发率低于对照组(P<0.05)。两组手术时间和治疗总有效率比较,差异无统计学意义(P>0.05)。结论对成人腹股沟疝患者,腹腔镜疝修补术手术时间与手术效果同开放式无张力疝修补术相当,但其具有创伤小、恢复快、术后疼痛轻、并发症及复发少等方面优势。Objective To compare the effect of laparoscopic hernia repair and open tension-free hernia repair in the treatment of adult inguinal hernia.Methods From January 2023 to March 2024,58 adult inguinal hernia patients admitted to the Community Health Service Center of Kouzhen Street Community in Laiwu District of Jinan City were selected and divided into observation group and control group by random number table method,29 cases in each group.The observation group received laparoscopic hernia repair,and the control group received open tension-free hernia repair.The perioperative indicators,surgical outcomes,pain levels[Visual Analogue Scale(VAS)],complications,and recurrence rates were compared between the two groups.Results The intraoperative blood loss in the observation group was less than that in the control group,and the postoperative exhaust time,first ambulation time,and hospitalization time were shorter in the observation group;at 12,24,and 72 hours postoperatively,the VAS scores in the observation group were lower than those in the control group;the incidence of postoperative complications and the recurrence rate after 3 months of follow-up in the observation group were lower than those in the control group(P<0.05).There were no statistically significant differences in operative time and overall treatment efficacy between the two groups(P>0.05).Conclusions For adult patients with inguinal hernia,laparoscopic hernia repair has comparable operative time and surgical outcomes to open tension-free hernia repair,but it offers advantages such as minimal trauma,faster recovery,less postoperative pain,and fewer complications and recurrences.
