摘　　要：目的分析基于远程管理的自动化腹膜透析(APD)在腹膜透析(PD)患者中的应用效果。方法选取2022年1月—2023年1月南方医科大学南方医院收治的100例PD患者,采用随机数字表法分为观察组和对照组各50例,两组均进行为期1年的随访。对照组采用持续非卧床腹膜透析(CAPD)治疗并实施常规随访管理,观察组采用APD治疗并在常规随访管理上实施远程管理模式。比较两组临床相关指标、透析充分性、并发症及自我管理能力。结果随访1年后,两组血肌酐、尿酸、血红蛋白、白蛋白、钾、24 h超滤量、24 h尿量、尿素清除指数及并发症总发生率比较,差异无统计学意义(P>0.05)。干预后,观察组换液技术操作、操作中异常情况处理、饮食管理、并发症监测、情绪管理及社会回归评分以及总分均高于干预前,且高于对照组,差异有统计学意义(P<0.05);对照组换液技术操作、饮食管理、并发症监测、情绪管理及社会回归评分以及总分均高于干预前,差异有统计学意义(P<0.05),但操作中异常情况处理评分与干预前比较,差异无统计学意义(P>0.05)。结论基于远程管理的APD能够提高PD患者的自我管理能力。Objective To analyze the application effect of automated peritoneal dialysis(APD)based on remote management in patients with peritoneal dialysis(PD).Methods A total of 100 patients with PD admitted to Nanfang Hospital Southern Medical University from January 2022 to January 2023 were selected and divided into observation group and control group with 50 cases in each group by random number table method.Both groups were followed up for 1 year.The control group was treated with continuous ambulatory peritoneal dialysis(CAPD)and carried out routine follow-up management,while the observation group was treated with APD and carried out remote management mode on routine follow-up management.Clinical indicators,dialysis adequacy,complication and self-management ability were compared between the two groups.Results After 1 year follow-up,there were no significant differences in serum creatinine,uric acid,hemoglobin,albumin and potassium levels,24 h ultrafiltration volume,24 h urine volume,kinetic ureaclearance index and the total incidence of complications between two groups(P>0.05).After intervention,the scores of fluid replacement technique operation,abnormal situation treatment in operation,diet management,complication monitoring,emotional management and social regression,and total scores in the observation group were higher than those before intervention,and higher than those in the control group,the differences were statistically significant(P<0.05).The scores of fluid replacement technique operation,diet management,complication monitoring,emotional management and social regression,and total scores in the control group were higher than those before intervention,and the differences were statistically significant(P<0.05);however,there was no significant difference in the treatment score of abnormal situation in operation compared with that before intervention(P>0.05).Conclusions APD based on remote management can effectively improve the self-management ability of PD patients.
