下咽癌术后并发症护理思维导图的构建

摘　　要：目的构建下咽癌术后并发症护理的思维导图,帮助护士快速掌握护理重点。方法总结分析2017年10月—2021年6月浙江大学医学院附属第一医院收治的100例行下咽癌扩大切除同时行锁骨上岛状皮瓣修复及胸大肌皮瓣修复术后出现并发症患者的护理经验并查阅相关文献等,构建下咽癌术后并发症护理思维导图。随机选取10例男性患者,采用随机数字表法分为对照组和试验组各5例。对照组由未接受思维导图培训的护士进行常规护理,试验组在常规护理基础上由接受思维导图培训的护士进行护理。结果试验组1例患者发生咽瘘,平均住院时间为15 d;对照组有4例患者出现并发症,其中咽瘘2例,吞咽障碍1例,吸入性肺炎1例,平均住院时间为28 d。试验组并发症发生率低于对照组,平均住院时间短于对照组。结论使用思维导图可以帮助护士更好地预见和处理下咽癌术后并发症,从而缩短住院时间,降低医疗成本,提高患者满意度。Objective To construct the mind mapping of postoperative complications nursing of hypopharyngeal cancer,so as to help nurses grasp the focus of nursing quickly.Methods To summarize and analyze the nursing experience of 100 patients with complications after extended resection of hypopharyngeal carcinoma and supraclavicular island flap repair and pectoralis major myocutaneous flap repair in the First Affiliated Hospital,Zhejiang University School of Medicine from October 2017 to June 2021,and to consult relevant literature,so as to construct the mind mapping of postoperative complications nursing of hypopharyngeal cancer.A total of 10 male patients were randomly selected and divided into control group and experimental group with 5 cases in each group by random number table method.The control group received routine nursing from nurses who did not receive mind mapping training,and the experimental group received nursing intervention from nurses who received mind mapping training on the basis of routine nursing.Results A total of 1 patient in the experimental group developed pharyngeal fistula,and the average hospital stay was 15 days.In the control group,4 patients developed complications,including 2 cases of pharyngeal fistula,1 case of dysphagia,and 1 case of aspiration pneumonia.The average length of hospitalization was 28 days.The complication rate of the experimental group was lower than that of the control group,and the average hospital stay was shorter than that of the control group.Conclusions The use of mind mapping can help nurses to predict and manage the complications of hypopharyngeal cancer better,so as to shorten hospital stay,reduce medical costs and improve patient satisfaction.
