病房护士对先天性心脏病术后患儿ICU过渡期护理体验的质性研究

摘　　要：目的深入了解病房护士对先天性心脏病术后患儿重症监护室(ICU)过渡期的护理体验。方法采用现象学研究方法,对2023年5—11月在上海市儿童医院采用目的抽样法选取的12名心外科病房护士进行半结构式深入访谈,并使用Colaizzi7步分析法进行数据收集及整理,分析提炼出主题。结果分析得出病房护士护理先天性心脏病术后ICU转出患儿体验的3个主题,分别为护理情感体验的矛盾性、护理认知体验不足、护理行为应对不足。结论在护理先天性心脏病术后ICU转出患儿过程中,需要重视护士的心理健康,促进身心健康,强化ICU过渡期护理专业知识及重症护理操作的同时,提倡家长参与式护理模式,促进其参与护理的意识和能力,并加强医护及多学科协作,通过制订先天性心脏病患儿ICU过渡期护理干预方案,完善相关流程,优化ICU过渡期管理。Objective To deeply understand the ward nurses'experience in nursing children who are transferred from ICU after congenital heart surgery.Methods Using the phenomenological research method,a semi-structured in-depth interview was conducted on 12 nurses in the cardiac surgery ward selected by the purpose sampling method in Shanghai Children's Hospital from May to November 2023,and the Colaizzi 7-step analysis method was used to collect and organize the data,analyze and extract the theme.Results A total of 3 themes of ward nurses'experience of transferring children out of ICU after congenital heart surgery were analyzed,which were the contradiction of nursing emotional experience,the lack of nursing cognitive experience and the lack of coping with nursing behavior.Conclusions In the process of nursing children with congenital heart disease after ICU transfer out,it is necessary to pay attention to the mental health of nurses,promote physical and mental health,strengthen professional knowledge of ICU transition care and intensive care operation,advocate the parental participatory nursing mode,promote the awareness and ability of parents to participate in nursing,strengthen the cooperation between doctors,nurses and multidisciplinary departments,and improve relevant processes and optimize ICU transition management through formulating the intervention plan of ICU transitional nursing for children with congenital heart diseases.
