隔附子饼灸对脾胃虚弱型脓毒症胃肠功能障碍的干预效果

摘　　要：目的 探讨隔附子饼灸疗法对脾胃虚弱型脓毒症胃肠功能障碍的干预效果。方法 选取2023年6-12月该院重症医学科收治的确诊为脾胃虚弱型脓毒症胃肠功能障碍患者72例,采用随机数字表法分为观察组和对照组,每组36例。对照组在常规医疗的同时遵医嘱给予常规艾灸治疗,观察组在常规医疗的同时给予隔附子饼灸治疗。2组患者干预周期均为7 d。比较2组患者干预前后急性生理学和慢性健康状况评价Ⅱ(APACHEⅡ)评分、急性胃肠损伤(AGI)分级、肠鸣音、腹腔内压、腹围、中医证候积分等变化,以评估干预效果。结果 2组患者干预前APACHEⅡ评分、AGI分级、肠鸣音、腹腔内压、腹围、中医证候积分比较,差异均无统计学意义(P>0.05);观察组患者干预后APACHEⅡ评分、AGI分级、肠鸣音、腹腔内压、腹围、中医证候积分均明显低于对照组,差异均有统计学意义(P<0.05)。结论 隔附子饼灸能有效改善脾胃虚弱型脓毒症胃肠功能障碍患者APACHEⅡ评分、AGI分级、肠鸣音、腹腔内压、腹围、中医证候积分等,疗效优于常规艾灸治疗。Objective To explore the intervention effect of aconite cake-separated moxibustion therapy on gastrointestinal dysfunction in sepsis with spleen-stomach deficiency.Methods A total of 72 patients with sepsis gastrointestinal dysfunction of spleen-stomach deficiency type admitted to the intensive care unit of our hospital from June to December 2023 were selected and divided into observation group and control group by random number table method,with 36 cases in each group.The control group was given routine moxibustion treatment according to the doctor′s advice while routine medical treatment,and the observation group was given aconite cake-separated moxibustion treatment while routine medical treatment.The intervention period was seven days in both groups.The changes of acute physiology and chronic health evaluationⅡ(APACHEⅡ)score,acute gastrointestinal injury(AGI)grade,bowel sounds,intra-abdominal pressure,abdominal circumference and TCM syndrome score before and after intervention were compared between the two groups to evaluate the intervention effect.Results There was no significant difference in APACHEⅡscore,AGI grade,bowel sounds,intra-abdominal pressure,abdominal circumference and TCM syndrome score between the two groups before intervention(P>0.05).After intervention,APACHEⅡscore,AGI grade,bowel sounds,intra-abdominal pressure,abdominal circumference and TCM syndrome score in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion Aconite cake-separated moxibustion can effectively improve APACHEⅡscore,AGI grade,bowel sounds,intra-abdominal pressure,abdominal circumference and TCM syndrome score in patients with sepsis gastrointestinal dysfunction of spleen-stomach deficiency type,and the curative effect is better than that of conventional moxibustion.
