摘 要:目的进行老年腰椎管狭窄症(LSS)神经源间歇性跛行(NIC)患者足底压力分析(COP),客观评价老年LSS NIC患者平衡及行走能力,验证COP的应用价值。方法回顾性分析2020年2月—2022年2月北京大兴区人民医院收治的40例老年LSS NIC患者(观察组)以及40例体检健康志愿者(对照组)的资料。采用COP系统进行静态足底平衡测试,评估前、后足面积,前、后足负荷占体质量比等指标值。动态分析前足触地(FFCP)、足跟触地(ICP)、全足触地(FFP)及前足离地(FFPOP)时足底压力中心速度。结果2组前、后足面积差异无统计学意义(P>0.05);观察组前足负荷占体质量比低于对照组(P<0.05),后足负荷占体质量比高于对照组(P<0.05);观察组FFCP、FFP足底压力中心速度值高于对照组(P<0.05),2组ICP、FFPOP足底压力中心速度值差异无统计学意义(P>0.05)。结论老年LSS NIC患者症状发作后站立时前后足重心分布可能不均匀,其平衡能力显著受损;可利用FFCP期和FFP期足底压力中心速度对其动态平衡功能进行客观评估,对该疾病的诊疗和功能评估具有一定的应用价值。Objective To analyze the plantar pressure(COP)in elderly patients with neurogenic intermittent claudication(NIC)caused by lumbar spinal stenosis(LSS),evaluate their balance and walking ability,and verifying the application value of COP.Methods A retrospective analysis was performed on 40 elderly LSS NIC patients(observation group)and 40 healthy volunteers(control group)admitted to People's Hospital of Daxing District from February 2020 to February 2022.The static plantar balance test was performed by a COP system.The areas of forefoot and hindfoot,the ratio of forefoot and hindfoot load to body weight and other indicators were measured.The COP center velocity during forefoot contact phase(FFCP),heel contact phase(ICP),full-foot contact phase(FFP)and forefoot push-off phase(FFPOP)were dynamically analyzed.Results There was no significant difference between the two groups in the areas of forefoot and hindfoot(P>0.05).The ratio of forefoot load to body weight in the observation group was significantly lower than that in the control group(P<0.05)and the ratio of hindfoot load to body weight was significantly higher than that in the control group(P<0.05).The values of COP center velocity during FFCP and FFP in the observation group were significantly higher than those in the control group(P<0.05),but there was no significant difference in the values of COP center velocity during ICP and FFPOP between the two groups(P>0.05).Conclusion After the onset of symptoms in elderly LSS NIC patients,the distribution of the center of gravity of forefoot and hindfoot may be uneven when standing,and their balance ability is significantly impaired.The COP center velocity during FFCP and FFP can be used to objectively evaluate their dynamic balance function.It has certain application value for the diagnosis,treatment and functional evaluation of this disease.
