摘 要:目的探究小切口冠状动脉搭桥术后合并肺部感染患者血清miR-146a、miR-19a水平表达与预后的关系。方法回顾性分析2020年6月至2023年6月西安交通大学第一附属医院收治的344例冠心病患者的临床资料。根据有无发生肺部感染分为感染组(n=120)和未感染组(n=224),根据预后情况分为预后不佳组(n=32)和预后良好组(n=312)。实时荧光定量PCR检测所有患者血清miR-146a及miR-19a表达水平。记录患者心肺功能:峰值氧脉搏、左室舒张末期内径(LVEDD)和左室射血分数(LVEF)。Pearson相关性分析患者心肺功能与血清miR-146a及miR-19a表达水平的相关性。结果感染组患者合并吸烟史的人数明显多于未感染组患者(P<0.05),其余临床资料比较,差异无统计学意义(P均>0.05)。感染组患者血清miR-146a及miR-19a表达水平明显高于未感染组(P均<0.05)。预后不佳患者血清miR-146a及miR-19a水平明显高于预后良好组(P均<0.05)。预后不佳组患者峰值氧脉搏、LVEF明显低于预后良好组,LVEDD明显大于预后良好组(P均<0.05)。miR-146a水平与峰值氧脉搏和LVEF呈负相关,与LVEDD呈正相关(P均<0.05);miR-19a与LVEF呈负相关,与LVEDD呈正相关(P均<0.05),与峰值氧脉搏间无显著相关性(P>0.05)。结论血清miR-146a及miR-19a表达水平与冠心病患者肺部感染发生与发展情况及预后相关,临床可通过检测其水平对患者是否发生肺部感染进行预测,对预后情况进行评估。Objective To explore the relationship between serum miR⁃146a,miR⁃19a and prognosis in patient with coronary heart disease(CHD)and lung infection.Methods A retrospective analysis was performed on the clinical data of 344 patients with CHD admitted to the hospital between June 2020 and June 2023.According to presence or absence of lung infection,they were divided into infection group(n=120)and non⁃infection group(n=224).According to prognosis,they were divided into poor prognosis group(32 cases)and good prognosis group(312 cases).The expression levels of serum miR⁃146a and miR⁃19a in all patients were detected by real⁃time fluorescent quantitative PCR(qRT⁃PCR).The cardiopulmonary function indexes were recorded,including peak oxygen pulse,left ventricular end⁃diastolic diameter(LVEDD)and left ventricular ejection fraction(LVEF).The correlation between cardiopulmonary function and serum miR⁃146a,miR⁃19a was analyzed by Pearson correlation analysis.Results The number of patients with smoking history in infection group was significantly more than that in non⁃infection group(P<0.05),but there was no significant difference in the other clinical data between the two groups(all P>0.05).The expression levels of serum miR⁃146a and miR⁃19a in infection group were significantly higher than those in non⁃infection group(all P<0.05).The expression levels of serum miR⁃146a and miR⁃19a in poor prognosis group were significantly higher than those in good prognosis group(all P<0.05).The peak oxygen pulse and LVEF in poor prognosis group were significantly lower than those in good prognosis group,while LVEDD was significantly longer than that in good prognosis group(all P<0.05).The level of miR⁃146a was negatively correlated with peak oxygen pulse and LVEF,while positively correlated with LVEDD(all P<0.05).The level of miR⁃19a was negatively correlated with LVEF,positively correlated with LVEDD(all P<0.05),but not significantly correlated with peak oxygen pulse(P>0.05).Conclusion The expression leve
