摘 要:目的:在尿毒症血液透析治疗中采取长期深静脉置管模式、动静脉内瘘模式,对比分析两种模式的临床应用价值。方法:选取本院2021年1月至2024年1月收治的80例尿毒症患者,均接受血透治疗,以随机数表法为小组划分重要依据,分为对照组、观察组,两组例数均为40例。其中对照组采取长期深静脉置管模式,观察组采取动静脉内瘘模式。对比两种置管模式的临床应用效果。结果:观察组治疗后血红蛋白(Hb)、白蛋白(ALB)水平均高于对照组,治疗后C反应蛋白(CRP)、白细胞介素-6(IL-6)水平、并发症发生率均低于对照组,差异有统计学意义(P<0.05)。对照、观察两组标准化蛋白分解代谢率(nPCR)、尿素氮(BUN)、尿酸(UA)、尿素氮清除率(Kt/V)、尿素降低率(URR)、肌酐(Cr)比较,差异无统计学意义(P>0.05)。结论:相比于长期深静脉置管模式,动静脉内瘘模式在尿毒症血透中应用效果显著,可减轻炎症反应,提高Hb、ALB水平,且并发症发生率较低,具备较高的安全性。Objective:To compare and analyze the clinical application value of long-term deep vein catheterization mode and arteriovenous fistula mode in the treatment of uremia hemodialysis.Method:80 patients with uremia admitted to our hospital from January 2021 to January 2024 were selected,all of whom received hemodialysis treatment.The random number table method was used as an important basis for group division,and they were divided into a control group and an observation group,with a total of 40 cases in both groups.The control group adopted a long-term deep vein catheterization mode,while the observation group adopted an arteriovenous fistula mode.Compare the clinical application effects of two catheterization modes.Result:After treatment,the levels of hemoglobin(Hb)and albumin(ALB)in the observation group were higher than those in the control group.The levels of C-reactive protein(CRP),interleukin-6(IL-6),and incidence of complications were lower than those in the control group,and the differences were statistically significant(P<0.05).Comparing and observing the standardized protein catabolism rate(nPCR),urea nitrogen(BUN),uric acid(UA),urea nitrogen clearance rate(Kt/V),urea reduction rate(URR),and creatinine(Cr)between two groups,there was no statistically significant difference(P>0.05).Conclusion:Compared with the long-term deep vein catheterization mode,the arteriovenous fistula mode has a significant effect in hemodialysis for uremia,which can reduce inflammatory reactions,improve Hb and ALB levels,and has a lower incidence of complications and higher safety.
