体外振动排痰结合全方位气道护理对COPD伴呼吸衰竭俯卧位通气患者动脉血气及并发症的影响

摘　　要：目的:探究体外振动排痰结合全方位气道护理对COPD伴呼吸衰竭俯卧位通气患者的动脉血气及并发症的应用效果。方法:纳入COPD伴呼吸衰竭俯卧位通气患者100例,于24年1月-24年8月入院,将符合纳排标准的100例样本,随机分组。对照组(n=50)、观察组(n=50)分别给予单一常规护理及联合全方位气道护理。两组均干预2周。比较两组的入院前、出院前1d的血气分析指标,肺功能指标,并发症的发生情况。结果:出院前1d,观察组的氧分压(PaO_(2))、血氧饱和度(SaO_(2))均高于对照组,而动脉二氧化碳分压(PaCO_(2))低于对照组(P<0.05)。出院前1d,观察组的第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC(一秒率) 高于对照组(P<0.05)。经数据显示,出院前1d,两组并发症对比有差异,其中观察组更低(对照组16.00%vs观察组2.00%)(P<0.05)结论:体外振动排痰结合全方位气道护理对COPD伴呼吸衰竭俯卧位通气患者效果显著,可明显改善患者血气指标,肺功能指标,减少并发症的出现,提高预后。Objective:To explore the application effect of extracorporeal vibration expectoration combined with comprehensive airway care on arterial blood gas and complications in prone ventilation patients with COPD and respiratory failure.Methods:100 cases of prone ventilation patients with COPD with respiratory failure were included and admitted to hospital from January to August,24.100 samples that met the standard of attrition were randomly grouped.Control group(n=50)and observation group(n=50)were given single routine nursing and combined comprehensive airway nursing,respectively.Both groups were treated for 2 weeks.Blood gas analysis indexes,lung function indexes and the occurrence of complications were compared between the two groups before admission and 1 day before discharge.Results:1 day before discharge,oxygen partial pressure(PaO_(2))and blood oxygen saturation(SaO_(2))in observation group were higher than those in control group,while arterial carbon dioxide partial pressure(PaCO_(2))was lower than those in control group(P<0.05).One day before discharge,the first second forced expiratory volume(FEV1),forced vital capacity(FVC)and FEV1/FVC(one-second rate)in the observation group were higher than those in the control group(P<0.05).The data showed that 1 day before discharge,there was a difference in complications between the two groups,and the observation group was lower(control group 16.00%vs observation group 2.00%)(P<0.05).Conclusion:Extracorporeal vibration expectoration combined with comprehensive airway care has a significant effect on prone ventilation patients with COPD and respiratory failure,which can significantly improve blood gas indexes and lung function indexes of patients,reduce the occurrence of complications,and improve prognosis.
