摘　　要：目的:分析罗哌卡因复合枸橼酸舒芬太尼对无痛分娩产妇应激反应和产程的影响。方法:选取2023年1月至2023年12月在本院分娩的80例产妇为研究对象。以信封法分为观察组与对照组,各40例,两组产妇均进行无痛分娩,对照组盐酸罗哌卡因麻醉,观察组于对照组基础上再复合舒芬太尼。对比两组产程、疼痛程度、应激指标、胎儿血气指标及Apgar评分、产妇泌乳功能。结果:观察组第一产程、第二产程、第三产程时间均低于对照组,但仅在第一产程、第二产程上有显著统计差异(P<0.05);镇痛后15min、镇痛后30min、分娩结束后2h、分娩结束后12h、分娩结束后24h,两组NRS评分均降低,且观察组更低(P<0.05);镇痛后1h、新生儿分娩后,两组Cor、NE均降低,且观察组更低(P<0.05);观察组胎儿PaCO_(2)更低,PaO_(2)、pH更高(P<0.05);观察组产妇泌乳开始时间更短,泌乳量更多(P<0.05)。结论:在产妇无痛分娩时,应用罗哌卡因复合枸橼酸舒芬太尼麻醉,能缩短产程,降低产妇分娩期间及分娩后疼痛水平,减轻机体应激反应,改善胎儿结局和增加产妇泌乳功能。Objective:To analyze the effect of ropivacaine combined with sufentanil citrate on stress response and labor process of parturients undergoing painless delivery.Methods:80 parturients who gave birth in our hospital from January 2023 to December 2023 were selected as the research objects.According to the envelope method,they were divided into observation group and control group,with 40 cases in each group.Both groups of women underwent painless delivery.The control group was anesthetized with ropivacaine hydrochloride,and the observation group was combined with sufentanil on the basis of the control group.The labor process,pain degree,stress index,fetal blood gas index,Apgar score and maternal lactation function were compared between the two groups.Results:The time of the first stage of labor,the second stage of labor and the third stage of labor in the observation group were lower than those in the control group,but there were differences only in the first stage of labor and the second stage of labor(P<0.05).15 min after analgesia,30 min after analgesia,2h after delivery,12h after delivery and 24h after delivery,the NRS scores of the two groups decreased,and those of the observation group were lower(P<0.05).1h after analgesia and after neonatal delivery,Cor and NE of the two groups decreased,and those of the observation group were lower(P<0.05).In the observation group,the fetal PaCO_(2) was lower,PaO_(2) and pH were higher(P<0.05).The maternal lactation start time in the observation group was shorter and the lactation volume was more(P<0.05).Conclusion:In the painless delivery of parturients,the application of ropivacaine combined with sufentanil citrate anesthesia can shorten the labor process,reduce the pain level during and after delivery,reduce the stress response of the body,improve the fetal outcome and increase the lactation function of parturients.
