摘　　要：目的探讨关节镜下双通道U形减压联合跟骨骨刺切除治疗慢性足跟疼痛的临床效果。方法选取2020年1月至2022年1月在泗洪县中医院骨科接受手术治疗的22例慢性足跟疼痛患者作为研究对象,所有患者接受关节镜下双通道U形减压联合跟骨骨刺切除术治疗。观察手术效果,以及随访期间有无并发症发生、疾病复发。术前、术后2个月采用视觉模拟评分(VAS)与美国足踝外科协会(AOFAS)评分系统评估患者的足跟疼痛程度及足部功能。结果22例患者均手术成功,跟踪随访12~15个月,平均(13.69±0.71)个月。术中、术后以及随访期间未发生神经血管损伤、切口感染及深静脉血栓形成等并发症,也未见疾病复发。术后2个月,患者的VAS评分明显低于术前,AOFAS评分明显高于术前,差异具有统计学意义(P<0.05)。结论关节镜下双通道U形减压联合跟骨骨刺切除术治疗慢性足跟疼痛的效果令人满意,可以有效缓解足跟疼痛,并恢复足部功能,值得临床应用及推广。Objective To investigate the clinical effect of arthroscopic double-channel U-shaped decompression combined with calcaneal spur resection in the treatment of chronic heel pain.Methods From January 2020 to January 2022,22 patients with chronic heel pain who underwent surgical treatment in the orthopedics department of Sihong TCM Hospital were selected as the research objects.All patients received arthroscopic double-channel U-shaped decompression combined with calcaneal spur resection.The surgical effect,and complications and disease recurrence during the follow-up period were observed.Visual Analogue Scale(VAS)and American Orthopaedic Foot and Ankle Society(AOFAS)scoring system were used to evaluate the degree of heel pain and foot function of patients before and 2 months after operation.Results The 22 patients underwent successful surgery and were followed up for 12-15 months,with an average of(13.69±0.71)months.No complications such as neurovascular injury,incision infection and deep vein thrombosis occurred during operation,after operation and during follow-up,and no recurrence of the disease was found.At 2 months after operation,the VAS score of the patients was significantly lower than that before operation,the AOFAS score was significantly higher than that before operation,and the differences were statistically significant(P<0.05).Conclusion Arthroscopic double-channel U-shaped decompression combined with calcaneal spur resection has satisfactory clinical effect,which can effectively relieve heel pain and restore foot function.It is worthy of clinical application and promotion.
