摘　　要：目的调查包皮环切术后换药过程中患者创面操作性疼痛现状,分析其影响因素,并探讨与状态焦虑、疼痛恐惧等负性情绪的相关性。方法采用便利抽样法对山西省5所三级甲等医院307例包皮环切术后换药患者,应用一般资料调查表、数字疼痛模拟量表、状态焦虑量表和疼痛恐惧问卷进行调查。结果包皮环切术后创面操作性疼痛得分为(4.62±2.48)分。患者状态焦虑、疼痛恐惧与创面操作性疼痛呈正相关(P<0.01)。年龄、术后有无并发症、换药次数、距上次换药时间、状态焦虑、疼痛恐惧是包皮环切术后创面操作性疼痛的重要影响因素(P<0.01)。结论包皮环切术后创面操作性疼痛程度较高,并与状态焦虑和疼痛恐惧关系密切,医护人员换药时,应充分关注患者疼痛问题及相关负性情绪,以期降低患者疼痛程度,促进伤口愈合。Objective To investigate the current situation of wound procedural pain in patients with dressing change after circumcision,identify influencing factors,and explore its relationship with negative emotions like state anxiety and fear of pain.Methods Convenience sampling was used to select 307 circumcision patients from five GradeⅢ,Class A hospitals in Shanxi province.Data were collected using a general data questionnaire,Numerical Rating Scale,State Anxiety Inventory,and Fear of Pain Questionnaire.Results The score of wound procedural pain after circumcision was(4.62±2.48).There was a positive correlation between anxiety and fear of pain and wound procedural pain(P<0.01).Age,postoperative complications,number of dressing changes,time since last dressing change,state anxiety,and fear of pain were important influencing factors of wound procedural pain after circumcision(P<0.01).Conclusion The degree of post-circumcision wound procedural pain is relatively high,and it is closely related to state anxiety and pain fear,so medical staff should pay full attention to the patient's pain problem and related negative emotions when changing dressings,in order to reduce the pain degree of the patient and promote wound healing.
