摘　　要：目的:探究临床药师参与型家庭医生团队干预在老年冠心病患者中的应用效果。方法:选取2021年4—5月上海市奉贤区奉浦街道社区卫生服务中心收治的106例冠心病患者作为研究对象,分为对照组(53例)和干预组(53例)。对照组给予常规家庭医生团队干预,干预组给予临床药师参与型家庭医生团队干预。比较两组患者用药依从性、血脂指标及血脂达标率。结果:干预组用药依从性高于对照组(P=0.014)。干预前,两组总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平比较,差异无统计学意义(P>0.05);干预后,两组TC、TG、LDL-C水平均降低,且干预组低于对照组(P<0.05);两组HDL-C水平升高,且干预组高于对照组(P<0.05)。干预组血脂达标率高于对照组(P=0.020)。结论:临床药师参与型家庭医生团队干预在老年冠心病患者中的应用效果显著,可改善血脂指标,提高患者用药依从性和血脂达标率。Objective:To investigate the application effect of clinical pharmacist participatory family doctor team intervention in elderly patients with coronary heart disease.Methods:A total of 106 patients with coronary heart disease admitted to Shanghai Fengxian District Fengpu Street Community Health Service Center from April 2021 to May 2021 were selected as the study subjects.They were divided into control group(53 cases)and intervention group(53 cases).The control group received conventional family doctor team intervention,and the intervention group received clinical pharmacist participatory family doctor team intervention.The medication compliance,blood lipid indexes and blood lipid compliance rate of the two groups were compared.Results:The medication compliance in the intervention group was higher than that in the control group(P=0.014).Before intervention,there was no statistically significant difference in the levels of total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)between the two groups(P>0.05).After intervention,the levels of TC,TG and LDL-C in the two groups decreased and the levels in the intervention group were lower than those in the control group(P<0.05).The levels of HDL-C in the two groups increased and the levels in the intervention group were higher than those in the control group(P<0.05).The blood lipid compliance rate in the intervention group was higher than that in the control group(P=0.020).Conclusion:The application effect of clinical pharmacist participatory family doctor team intervention in elderly patients with coronary heart disease is significant,which can improve blood lipid indexes and increase medication compliance and blood lipid compliance rate of patients.
