摘　　要：目的:分析无创正压通气(NIPPV)联合电子支气管镜肺泡灌洗治疗慢性阻塞性肺疾病伴呼吸衰竭的临床效果。方法:将2022年2月—2023年2月佛山市禅城区人民医院南庄医院收治的慢性阻塞性肺疾病伴呼吸衰竭患者60例以随机数字表法分为对照组和观察组,各30例。对照组实施NIPPV治疗,观察组在对照组基础上联合电子支气管镜肺泡灌洗治疗。对比两组治疗效果。结果:观察组治疗总有效率高于对照组(P<0.001)。治疗后,两组经皮血氧饱和度、动脉血氧分压、第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC提高,且观察组高于对照组(P<0.05)。治疗后,两组咳嗽、咳痰评分下降,且观察组均低于对照组(P<0.05)。结论:NIPPV联合电子支气管镜肺泡灌洗治疗慢性阻塞性肺疾病伴呼吸衰竭的临床效果显著,能够改善患者肺功能、血气分析指标,减轻症状。Objective:To analyze the clinical effect of non-invasive positive pressure ventilation(NIPPV)combined with electronic bronchoscopy bronchoalveolar lavage in treatment of chronic obstructive pulmonary disease with respiratory failure.Methods:Sixty patients with chronic obstructive pulmonary disease and respiratory failure who were admitted to Nanzhuang Hospital of Foshan Chancheng People's Hospital from February 2022 to February 2023 were divided into a control group and an observation group using a random number table method,with 30 cases in each group.The control group received NIPPV treatment,while the observation group received electronic bronchoscopy bronchoalveolar lavage on the basis of control group.The treatment effects were compared between two groups.Results:The overall response rate of treatment in the observation group was higher than that in the control group(P<0.001).After treatment,the transcutaneous oxygen saturation,arterial partial pressure of oxygen,forced expiratory volume in 1 second(FEV_(1)),forced vital capacity(FVC)and FEV_(1)/FVC in the two groups increased,and these parameters in the observation group were higher than those in the control group(P<0.05).After treatment,the scores of cough and sputum in the two groups decreased,and the scores in the observation group were lower than those in the control group(P<0.05).Conclusion:NIPPV combined with electronic bronchoscopy bronchoalveolar lavage in the treatment of chronic obstructive pulmonary disease with respiratory failure has significant clinical effects,and can improve lung function and blood gas analysis indexes of patients,and alleviate symptoms.
