摘  要:目的:探讨IgA肾病患者血脂水平与T细胞受体保守域α链基因(TCRCα)-575A/G基因多态性的关系。方法:采用聚合酶链反应-限制性片段长度多态性分析法和聚合酶链反应产物直接测序法鉴定IgA肾病患者基因型,检测IgA肾病患者甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)水平,计算TG/HDL-C。统计IgA肾病患者TCRCα-575基因型分布;统计不同TCRCα-575基因型患者TG/HDL-C、TG、HDL-C水平;根据TG/HDL-C结果,将患者分为TG/HDL-C高组(≥1.385)及TG/HDL-C低组(<1.385),比较两组TCRCα-575基因型分布情况;根据TG水平将患者分为TG异常组(≥1.7 mmol/l)及TG正常组(<1.7 mmol/L),比较两组TCRCα-575基因型分布情况。结果:261例IgA肾病患者检出AA基因型48例、GG基因型90例和AG基因型123例。不同TCRCα-575基因型患者TG/HDL-C、TG水平比较,AA基因型患者<GG基因型患者<AG基因型患者,差异有统计学意义(P<0.05);不同TCRCα-575基因型患者HDL-C水平比较,无统计学差异(P>0.05)。TG/HDL-C高组TCRCα-575 AA基因型占比(8.1%)、GG基因型占比(31.4%)低于TG/HDL-C低组,AG基因型占比(60.5%)高于TG/HDL-C低组,差异有统计学意义(P<0.05)。TG异常组TCRCα-575 AA基因型占比(7.4%)、GG基因型占比(32.1%)低于TG正常组,AG基因型占比(60.5%)高于TG正常组,差异有统计学意义(P<0.05)。结论:IgA肾病患者血脂水平与TCRCα-575A/G基因型相关,TG水平、TG/HDL-C异常者多为AG基因型,TCRCα-575A/G基因可能通过调控免疫应答、炎性反应间接影响机体脂质代谢,但其对血脂水平的具体作用机制还需临床工作者开展大量分子实验进行研究。Objective:To explore the relationship between blood lipid levels and T cell receptor conserved domainαchain gene(TCRCα)-575A/G gene polymorphism in IgA nephropathy patients.Methods:The genotypes of IgA nephropathy patients were identified by polymerase chain reaction-restriction fragment length polymorphism analysis and direct sequencing of polymerase chain reaction products.The levels of triglyceride(TG)and high density lipoprotein cholesterol(HDL-C)in IgA nephropathy patients were detected.The TG/HDL-C ratio was calculated.The distribution of TCRCα-575 genotypes in IgA nephropathy patients was counted.The levels of TG/HDL-C,TG and HDL-C in patients with different TCRCα-575 genotypes were counted.According to the results of TG/HDL-C,the patients were divided into high TG/HDL-C group(≥1.385)and low TG/HDL-C group(<1.385).The distribution of TCRCα-575 genotypes in these two groups was compared.According to TG levels,the patients were divided into abnormal TG group(≥1.7 mmol/l)and normal TG group(<1.7 mmol/L).The distribution of TCRCα-575 genotypes in these two groups was compared.Results:Among the 261 patients with IgA nephropathy,48 cases of AA genotype,90 cases of GG genotype and 123 cases of AG genotype were detected.The levels of TG/HDL-C and TG in patients with different TCRCα-575 genotypes were compared and ranked as follows:patients of AA genotype<patients of GG genotype<patients of AG genotype,and the differences were statistically significant(P<0.05).There was no statistically difference in HDL-C levels among patients with different TCRCα-575 genotypes(P>0.05).The proportions of TCRCα-575 AA genotype(8.1%)and GG genotype(31.4%)in the high TG/HDL-C group were lower than those in the low TG/HDL-C group,while the proportion of AG genotype(60.5%)in the high TG/HDL-C group was higher than that in the low TG/HDL-C group,and the difference was statistically significant(P<0.05).The proportions of TCRCα-575 AA genotype(7.4%)and GG genotype(32.1%)in the abnormal TG group were lower than those in the n
