摘 要:目的 分析不同屈光状态、不同民族近视患者角膜光密度(COD)差异,探讨COD与角膜形态学参数的相关性。方法 2020年6月—2021年8月新疆维吾尔自治区人民医院诊治近视患者150例(150眼,均为右眼),根据屈光状态分为低度近视组(>-3.00~-0.50 D)24例(24眼)、中度近视组(>-6.00~-3.00 D)76例(76眼)和高度近视组(≤-6.00 D)50例(50眼)。应用Pentacam 70700眼前节分析系统测量角膜形态学参数(角膜中央厚度、角膜内皮细胞密度、六角形细胞比例),测定角膜不同层面(前层、中央层、后层、全层)、不同直径范围(以角膜中心点为参照,0~2、>2~6、>6~10、>10~12及0~12 mm)COD值。比较不同屈光状态及不同民族近视患者角膜各层面、各直径范围COD值;采用Pearson相关法分析不同屈光状态近视患者角膜各层面COD值与角膜形态学参数的相关性。结果 3组中央层、后层、全层各直径范围及角膜前层0~2、>2~6、>10~12 mm直径范围COD值比较差异均无统计学意义(F=0.288~2.499,P均>0.05);3组角膜前层>6~10 mm直径范围COD值比较差异有统计学意义(F=3.431,P=0.035),低度近视组(22.23±3.67)高于中度近视组(19.93±3.69)、高度近视组(20.50±3.89)(P<0.05),中度近视组与高度近视组比较差异无统计学意义(P>0.05)。3组同一直径范围COD值在角膜前层、后层、中央层、全层均依次降低(P<0.05),同一角膜层面COD值在>10~12 mm直径范围最高。汉族近视患者角膜前层、中央层、后层及全层0~12 mm直径范围COD值(22.57±2.42、15.83±1.43、17.00±1.55、12.63±1.11)均高于维吾尔族患者(21.29±2.52、14.94±1.35、15.98±1.49、12.03±1.14)(t=2.985~3.902,P均<0.05)。低度近视组中,角膜前层、中央层、后层、全层COD与角膜内皮细胞密度均呈负相关(r=-0.495,P=0.014;r=-0.488,P=0.016;r=-0.494,P=0.014;r=-0.509,P=0.011);中度近视组中,角膜前层、中央层、后层、全层COD与角膜中央厚度、角膜内皮细胞密度、六角�Objective To analyze the difference of corneal optical density(COD)in myopic patients of different refractive statuses and different ethnic groups,and to explore the correlation between COD and corneal morphological parameters.Methods Totally 150 myopic patients(150 right eyes)were treated in People's Hospital of Xinjiang Uygur Autonomous Region from June 2020 to August 2021,and were divided into the low myopia group(>-3.00 to-0.50 D,24 patients,24 eyes),moderate myopia group(>-6.00 to-3.00 D,76 patients,76 eyes)and high myopia group(≤-6.00 D,50 paitents,50 eyes)according to the refractive statuses.The corneal morphological parameters(corneal central thickness,corneal endothelial cell density,proportion of hexagonal cells)were measured by Pentacam 70700 anterior segment analysis system.The COD values of different corneal layers(anterior layer,central layer,posterior layer,full layer)and diameters ranges with reference to central point of cornea(0-2,>2-6,>6-10,>10-12,0-12 mm)were measured.The COD values of myopic patients with different refractive statuses and different ethnic groups were compared at different corneal layers and diameter ranges.The correlations between COD values of different corneal layers and corneal morphological parameters in myopia patients with different refractive statuses were analyzed by Pearson correlation method.Results The COD values in central layer,posterior layer and full layer of each diameter range and anterior layer of diameter ranges of 0-2,>2-6 and>10-12 mm showed no significant differences in three groups(F=0.288-2.499,all P values>0.05).The COD value in anterior layer of diameter range of>6-10 mm showed significant difference in three groups(F=3.431,P=0.035),was higher in the low myopia group(22.23±3.67)than that in the moderate myopia group(19.93±3.69)and the high myopia group(20.50±3.89)(P<0.05),and showed no significant difference between the moderate myopia group and the high myopia group(P>0.05).The COD values in the anterior layer,posterior layer,central layer and
