丹芪偏瘫胶囊联合吞咽治疗仪治疗脑卒中后吞咽障碍的效果及对吞咽功能和预后的影响

摘　　要：目的观察丹芪偏瘫胶囊联合吞咽治疗仪治疗脑卒中后吞咽障碍的效果及对吞咽功能和预后的影响。方法选取2022年1月—2023年6月长沙市中心医院/南华大学附属长沙中心医院收治的脑卒中后吞咽障碍患者90例,采用随机数字表法分为观察组与对照组,各45例。在常规对症治疗基础上,对照组采用吞咽治疗仪治疗,观察组在对照组基础上给予丹芪偏瘫胶囊治疗,2组均治疗3个月。比较2组临床疗效,治疗前后相关量表[美国国立卫生研究院卒中量表(NIHSS)、标准吞咽功能评定量表(SSA)、功能性经口摄食量表(FOIS)]评分、营养指标,并发症发生率。结果观察组治疗总有效率高于对照组(97.78%vs.80.00%,χ^(2)=7.200,P=0.007)。治疗3个月后,2组NIHSS、SSA评分低于治疗前,FIOS评分高于治疗前,且观察组降低/升高幅度大于对照组(P<0.01);2组三酰甘油、白蛋白、血红蛋白水平高于治疗前,且观察组高于对照组(P<0.01)。观察组并发症总发生率低于对照组(4.44%vs.20.00%,χ^(2)=5.075,P=0.024)。结论丹芪偏瘫胶囊联合吞咽治疗仪治疗脑卒中后吞咽障碍的效果显著,可提高患者摄食能力、吞咽功能,有效改善患者的神经功能及营养状态,且可降低并发症发生率。Objective To observe the effect of Danqi Piantan capsules combined with swallowing apparatus on dysphagia after stroke and its influence on swallowing function and prognosis.Methods A total of 90 patients with dysphagia after stroke admitted to Changsha Central Hospital/Changsha Central Hospital Affiliated to University of South China from January 2022 to June 2023 were selected and divided into the observation group and the control group by random number table method,with 45 cases in each group.On the basis of conventional symptomatic treatment,the control group was treated with swallowing therapy apparatus,and the observation group was treated with Danqi Piantan capsules on the basis of the control group.Both groups were treated for 3 months.The clinical efficacy,the scores of relevant scales(NIHSS,SSA,FOIS)and nutritional indexes before and after treatment,as well as the incidence of complications were compared between the two groups.Results The total effective rate of observation group was higher than that of control group(97.78%vs.80.00%,χ^(2)=7.200,P=0.007).After 3 months of treatment,NIHSS and SSA scores in two groups were lower than before treatment,FIOS scores were higher than before treatment,and the decrease/increase of the observation group was greater than that of the control group(P<0.01).The levels of triacylglycerol,albumin and hemoglobin in two groups were higher than before treatment,and the observation group were higher than those of the control group(P<0.01).The total complication rate of the observation group was lower than that of the control group(4.44%vs.20.00%,χ^(2)=5.075,P=0.024).Conclusion Danqi Piantan capsule combined with swallowing apparatus has a significant effect on the treatment of dysphagia after stroke,which can improve the feeding ability and swallowing function of patients,effectively improve the neurological function and nutritional status of patients,and reduce the incidence of complications.
