归脾汤联合盐酸丁螺环酮片治疗心脾两虚型焦虑障碍的效果

摘　　要：目的探究归脾汤联合盐酸丁螺环酮片治疗心脾两虚型焦虑障碍的效果。方法选取2022年5—12月赣州市第三人民医院收治的60例心脾两虚型焦虑障碍患者作为研究对象,采用随机数字表法为对照组与研究组,各30例。对照组给予盐酸丁螺环酮片治疗,观察组在对照组基础上联合归脾汤治疗。比较两组证候积分、焦虑与抑郁评分、临床症状评分、炎症因子水平、激素指标及临床疗效。结果治疗后8周,研究组证候积分(善思多虑、失眠多梦、心悸、食欲不振、神疲乏力、头晕、自汗、善太息、面色萎黄)、汉密尔顿焦虑量表、焦虑自评量表、汉密尔顿抑郁量表、抑郁自评量表及临床总体印象量表评分均低于对照组,差异有统计学意义(P<0.05)。治疗后8周,研究组C反应蛋白、白细胞介素-1α、白细胞介素-6、白细胞介素-12、干扰素γ、促肾上腺皮质激素、皮质醇、促甲状腺激素水平均低于对照组,游离三碘甲状腺原氨酸、游离甲状腺素水平均高于对照组,差异有统计学意义(P<0.05)。治疗后1、2、4、6、8周,两组治疗副反应量表评分比较差异无统计学意义。研究组治疗总有效率为90.00%,高于对照组的66.67%,差异有统计学意义(P<0.05)。结论归脾汤联合盐酸丁螺环酮片治疗心脾两虚型焦虑障碍效果显著,可明显改善患者临床症状,缓解其焦虑、抑郁情绪,其机制可能与抑制机体炎症反应、调节激素水平有关。Objective To explore the effect of Guipi decoction combined with buspirone hydrochloride tablet in the treatment of anxiety disorder with heart and spleen deficiency.Methods 60 patients with anxiety disorder with heart and spleen deficiency treated in the Third People's Hospital of Ganzhou from May to December 2022 were selected as the study subjects,they were divided into the control group and the study group by random number table method,with 30 cases in each group.The control group was treated with buspirone hydrochloride tablets,and the observation group was treated with Guipi decoction on the basis of the control group.The syndrome score,anxiety and depression score,clinical symptom score,inflammatory factor level,hormone index and clinical efficacy were compared between the two groups.Results At 8 weeks after treatment,the syndrome scores(hyperthinking,insomnia and dreaminess,palpitation,loss of appetite,fatigue,dizziness,spontaneous sweating,hyperbreath,yellow face),Hamilton anxiety scale,self-rating anxiety scale,Hamilton depression scale,self-rating depression scale and clinical general impression scale in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).At 8 weeks after treatment,the levels of C-reactive protein,interleukin-1α,interleukin-6,interleukin-12,interferon-γ,adrenocorticotropin,cortisol and thyrotropin in the study group were lower than those in the control group,and the levels of free triiodothyronine and free thyroxine were higher than those in the control group,the differences were statistically significant(P<0.05).At 1,2,4,6 and 8 weeks after treatment,there was no significant difference in treatment emergent symptom scale scores between the two groups.The total effective rate in the study group was 90.00%,which was higher than 66.67%in the control group,the difference was statistically significant(P<0.05).Conclusion Guipi decoction combined with buspirone hydrochloride tablets has a remarkable effect in the treatment of he
