摘　　要：目的探究自动弹力线痔疮套扎术(automatic rubber band ligation of hemorrhoids-4,RPH-4)联合外痔切除及皮桥下静脉剥除应用于环状混合痔患者中的疗效与安全性。方法选取2021年4月至2022年4月广东省人民医院赣州医院收治的86例环状混合痔患者作为研究对象,按照随机抽签法分为两组,各43例。对照组行传统混合痔内扎外剥术治疗,研究组行RPH-4联合外痔切除及皮桥下静脉剥除治疗。比较两组围手术期相关指标、炎症因子[WBC、C反应蛋白(C-reactive protein,CRP)、中性粒细胞百分比(neutrophilic granulocyte,NE)]水平、排便功能及并发症发生情况。结果研究组手术时间及愈合时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);研究组治疗总有效率为95.35%,高于对照组的79.07%,差异有统计学意义(P<0.05);术后,两组WBC及CRP、NE水平均高于术前,但研究组低于对照组,差异有统计学意义(P<0.05);研究组排便功能总优良率为88.37%,高于对照组的69.77%,差异有统计学意义(P<0.05);研究组并发症发生率为4.65%,低于对照组的20.93%,差异有统计学意义(P<0.05)。结论RPH-4联合外痔切除及皮桥下静脉剥除治疗环状混合痔效果较佳,可促进机体恢复,改善炎症因子,恢复排便功能,安全可靠。Objective To explore the efficacy and safety of automatic rubber band ligation of hemorrhoids-4(RPH-4)combined with external hemorrhoidectomy resection and skin bridge vein stripping in the treatment of annular mixed hemorrhoids.Methods A total of 86 patients with annular mixed hemorrhoids admitted to Ganzhou Hospital of Guangdong Provincial People's Hospital from April 2021 to April 2022 were selected as the research subjects,they were divided into the two groups according to random drawing method,with 43 cases in each group.The control group was treated with traditional internal ligation and external stripping of mixed hemorrhoids,and the study group was treated with RPH-4 combined with external hemorrhoids resection and skin bridge vein stripping.The perioperative related indexes,inflammatory factors(WBC,C-reactive protein[CRP],neutrophilic granulocyte[NE])level,defecation function and postoperative complications were compared between the two groups.Results The operation and healing time in the study group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,the differences were statistically significant(P<0.05).The total effective rate of treatment in the study group was 95.35%,which was higher than 79.07%in the control group,and the difference was statistically significant(P<0.05).After operation,the levels of WBC,CRP and NE of the two groups were higher than those before operation,but the study group were lower than the control group,the differences were statistically significant(P<0.05).The total excellent and good rate of defecation function in the study group was 88.37%,which was higher than 69.77%in the control group,the difference was statistically significant(P<0.05).The incidence of complications in the study group was 4.65%,which was lower than 20.93%in the control group,the difference was statistically significant(P<0.05).Conclusion RPH-4 combined with external hemorrhoids resection and skin bridge vein stripping is effective in the
