闭合复位经皮空心螺钉内固定术治疗踝关节骨折的临床疗效及安全性评价

摘　　要：目的探讨闭合复位经皮空心螺钉内固定术(percutaneous cannulated screw internal fixation,MINI)治疗踝关节骨折的临床疗效及安全性。方法选取2020年2月至2023年2月文登整骨烟台医院收治的60例踝关节骨折患者作为研究对象,采用随机数字表法分为对照组与观察组,每组30例。对照组行切开复位内固定术(open reduction and internal fixation,ORIF),观察组行MINI。比较两组临床疗效、手术相关指标、踝关节功能恢复情况、并发症发生率、健康调查表36(36-item short from health survey,SF-36)评分。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组手术时间、骨折愈合时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。观察组踝关节功能恢复优良率高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。术后3个月,两组生理功能、生理职能、情感职能、精神健康、活力、社会功能、躯体疼痛、总体健康评分均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论MINI治疗踝关节骨折患者的临床疗效显著,可改善患者踝关节功能,具有创伤小、恢复快速、并发症发生风险低等优势,且可提高患者的生命质量。Objective To explore the clinical efficacy and safety of closed reduction and percutaneous cannulated screw internal fixation(MINI)in patients with ankle joint fracture.Methods 60 patients with ankle joint fracture admitted to Yantai Hospital of Wendeng Orthopaedics&Traumatology from February 2020 to February 2023 were selected as the study subjects,and they were divided into the control group and the observation group by random number table method,with 30 cases in each group.The control group received open reduction and internal fixation(ORIF),and the observation group received MINI.The clinical efficacy,surgery-related indexes,ankle joint function recovery,complication rate and 36-item short from health survey(SF-36)scores were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The operation time,fracture healing time and hospitaliztion time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,and the differences were statistically significant(P<0.05).The excellent and good rate of ankle joint function recovery in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).At 3 mothes after surgery,the scores of physical function,physical function,emotional function,mental health,vitality,social function,body pain and overall health of the two groups were higher than those before surgery,and the observation group was higher than the control group,and the differences were statistically significant(P<0.05).Conclusion MINI has significant clinical effect in the treatment of ankle joint fracture patients,can improve the ankle function of patients,has the advantages of less trauma
